Effects of Ischaemia on the Neuroretina and Retinal Blood Vessels

Abstract
Identification of the intracellular signal transduction pathways activated in retinal ischaemia may be important in revealing novel pharmacological targets. The retinal blood vessels are key organs in circulatory failure, and in this work the retinal vasculature was therefore examined separately from the neuroretina. The porcine eye has a typical primate-like architecture and is similar to the human eye regarding its size and retinal blood supply. The retinal blood vessels are key organs in circulatory failure, and in this work the retinal vasculature was therefore examined separately from the neuroretina. The porcine eye has a typical primate-like architecture and is similar to the human eye regarding its size and retinal blood supply. The arteries in the porcine eye are large enough to allow dissection, and can be used for both functional and molecular analysis. Therefore, the first aim of this work was to set up and evaluate a porcine model of pressure-induced retinal ischaemia-reperfusion injury. The second aim was to study the intracellular signal transduction pathways activated in retinal ischaemia, including mitogen-activated protein kinases (MAPKs), protein kinase C (PKC), tumour necrosis factor (TNF), hypoxia-inducible factor (HIF) and vascular endothelial growth factor (VEGF). The results show that the porcine model of retinal ischaemia-reperfusion was successfully established, and that retinal blood vessels and the neuroretina could be studied separately. The retinal circulation was completely obstructed at an intra ocular pressure of 80 mmHg, and fluorescein angiography during reperfusion showed how the circulation was restored. Changes were seen in multifocal electroretinograms following the ischaemic insult, showing decreased amplitudes and increased implicit times. Pyknotic cell nuclei count, TUNEL-positive cells and glial fibrillary acidic protein mRNA expression were increased as a result of ischaemia, suggesting retinal injury and glial cell activation. The expression of signalling pathways including MAPKs, PKC, TNF, HIF and VEGF was altered in both the neuroretina and retinal arteries, in a way that is typical of ischaemia. These are intracellular signalling molecules that may be important in the development of retinal injury following ischaemia, and may thus be interesting targets for the development of pharmacological therapeutic agents. Oftast är en kärlnybildning bra, men inte i näthinnan. De nya blodkärlen fungerar inte bra, utan växer onormalt och läcker vilket i längden kan leda till synnedsättning. Detta sker bl.a. hos diabetiker och är en av de vanligaste

orsakerna till blindhet i världen idag.



Patienter där näthinnan har drabbats av syrebrist (retinal ischemi) behandlas idag med laser. Med hjälp av laser bränner man bort stora delar av den skadade näthinnan för att förhindra att nya blodkärl bildas. Detta är således en mycket grov behandlingsmetod som slår ut stora delar av näthinnan. Ofta får patienter som genomgått laserbehandling minskat synfält och försämrat mörkerseende.



Syftet med min avhandling var att kartlägga några signalvägar, på cellnivå, som är involverade i utvecklingen av skador på näthinnan och dess blodkärl till följd av syrebrist. Ökad kunskap inom detta område leder förhoppningsvis till att vi i framtiden kan behandla syrebrist i näthinnan med läkemedel och därmed förbättra synen hos dessa patienter.



I studie I skapade vi ischemi i näthinnan på grisar genom att höja trycket i ögat. Om trycket i ögat höjs så kommer de ytliga blodkärlen tryckas ihop vilket gör att blodflödet till näthinnan stoppas. Vi kom fram till att gris är ett bra försöksdjur för att studera syrebrist i näthinnan. Grisögon är mycket lika människans ögon och har ett fint kärlträd. Vi tog bilder av näthinnan för att visa hur blodflödet upphör när vi höjer trycket i ögat för att trycka ihop kärlen och skapa syrebrist. Funktionen

i näthinnan kunde vi mäta med hjälp av en undersökning av ögat som mäter det elektriska svaret i näthinnan när man stimulerar med ljus (mfERG). Funktionen inäthinnan minskade på ett sätt som liknar det man ser hos patienter som har fått en blodpropp i näthinnan.



Forskning om de cellulära signalvägar som är involverade i utvecklingen av ischemi går snabbt framåt vad gäller andra sjukdomar som beror på syrebrist (ischemi) t ex hjärtinfarkt och stroke. I studie II-V undersökte vi hur signalvägarna MAPK, PKC, TNF, HIF och VEGF förändrades i näthinnan och dess blodkärl vid ischemi. Vi fann att vid ischemi så ökades MAPK i mängd och aktivitet, samt att PKC minskade. TNFα och dess receptorer, HIF-1α och VEGF visade sig vara ökade i glaskroppen, näthinna och näthinnans blodkärl vid ischemi. Detta antyder att ischemin har satt igång en serie av händelser som kan leda till skada i näthinnan.



Mer forskning behövs för att i detalj kartlägga cellernas signaleringsvägar. Vi hoppas kunna medverka till att ta fram ett effektivt och säkert läkemedel som kan hindra den skada som uppkommer när näthinnan drabbas av syrebrist. (Less)
