Streptococci constitute a heterogeneous group of Gram-positive bacteria, divided into different serological groups on the basis of antigenic differences in a cell wall-associated carbohydrate. This thesis describes related surface proteins expressed by bacteria in two of these groups, the group B streptococcus (GBS) and the group A streptococcus (GAS). GBS is an encapsulated bacterium that is the major cause of invasive bacterial infections in the neonatal period. ABSTRACT



Streptococci constitute a heterogeneous group of Gram-positive bacteria, divided into different serological groups on the basis of antigenic differences in a cell wall-associated carbohydrate. This thesis describes related surface proteins expressed by bacteria in two of these groups, the group B streptococcus (GBS) and the group A streptococcus (GAS). GBS is an encapsulated bacterium that is the major cause of invasive bacterial infections in the neonatal period. We have identified and purified a novel cell surface protein, Rib, which is expressed by many GBS strains causing invasive infections. Protein Rib confers protective immunity in a mouse model, making it of interest for analysis of pathogenetic mechanisms and for vaccine development. Sequence analysis demonstrated that the sequence of Rib is related to that of a previously described GBS protein, the alpha protein. Both of these proteins have exceptionally long signal peptides and their sequences are extremely repetitive. Although the two proteins show extensive amino acid residue identity they do not cross-react immunologically. Rib and alpha share several properties i.e. size variation between strains and protease resistance, and they show a characteristic laddering pattern when analyzed in Western blots, due to hydrolysis of acid-labile Asp-Pro bonds. These data show that the Rib and alfa proteins are members of a novel family of streptococcal surface proteins with unusual repetitive structure. R28 is a surface protein expressed by some strains of group A streptococci (Streptococcus pyogenes). The R28 protein cross-reacts immunologically with the Rib protein of GBS, and sequence analysis showed that R28 is a member of the same family of repetitive proteins as Rib and alpha. R28 promotes adhesion to human epithelial cells, and has the important property to elicit protective immunity. Interestingly, the R28 and Rib proteins, which are expressed by different bacterial species, confer cross-protection, i.e. immunization with R28 protects against Rib-expressing strains, and vice versa. Sjukdomsförloppet är ofta våldsamt och livshotande. Sedan 1970-talet har det skett en betydande ökning av antalet barn, som insjuknar på grund av GBS infektion, enligt rapporter från skilda delar av världen. Dessutom insjuknar även en del vuxna i allvarliga GBS infektioner . C:a 20-30% av alla kvinnor är koloniserade med GBS i vagina. När en gravid kvinna bär på GBS, kan fostret utsättas för GBS redan före förlossningen eller också under barnets passage genom födelsekanalen. Trots detta insjuknar emellertid endast c:a 0,2% av alla nyfödda barn i GBS infektion. Skälet att just dessa barn insjuknar är ej känt. De flesta av de GBS-infekterade barnen är sjuka redan vid födelsen eller insjuknar under de första 7 dagarna, s.k. "early onset" sjukdom. Andra barn insjuknar under de tre första månaderna, s.k. "late onset" sjukdom. Dödligheten är 10-15% för "early onset" och 2-6% för "late onset". Dessutom kan neurologiska skador förekomma hos de barn, som överlever. GBS är kapselförsedda bakterier, som är indelade i olika typer på grundval av polysackaridkapselns struktur. Kapseln har visat sig vara viktig för streptokockernas sjukdomsalstrande förmåga. Renframställd polysackarid-kapsel kan ge upphov till skyddande immunitet i djurförsök, dock endast efter att först ha kopplats kemiskt till ett s.k. bärarprotein. Dessutom uttrycker GBS ytproteiner, som troligen är viktiga för bakteriens förmåga att orsaka sjukdom. Både polysackariderna och ytproteinerna är föremål för studier med syftet att utveckla ett vaccin mot GBS. Huvuddelen av denna avhandling behandlar ytproteiner hos GBS. Grupp A streptokocker (GAS, Streptoccous pyogenes) Grupp A streptokocker, eller på latin Streptococcus pyogenes, är en av de vanligaste sjukdomsframkallande bakterierna hos människa. Denna bakterie kan ge upphov till en rad sjukdomar, av vilka de vanligaste är halsfluss och hudinfektionen svinkoppor (impetigo). Dessutom orsakar S. pyogenes scharlakansfeber (scarlatina) och rosfeber (erysipelas), samt ger upphov till allvarliga och livshotande tillstånd såsom hastigt framskridande destruktion av kroppens mjukdelar, blodförgiftning och chock. Dessa allvarliga sjukdomstillstånd har i pressen gett S. pyogenes namnet "mördarbakterien". Liksom GBS uttrycker S. pyogenes ytproteiner som är viktiga för bakteriens förmåga att orsaka sjukdom.



Kortfattad sammanfattning av delarbetena I-V Studier av GBS. Vi har identifierat, isolerat och renat ett ytprotein, Rib, vilket uttrycks hos de flesta av de GBS-stammar som orsakar allvarliga infektioner (arbete I). Andra stammar av GBS, som inte uttrycker Rib uttrycker ofta ett annat ytprotein, alfa. Sammantaget finns Rib eller alfa hos c:a. 90% av alla GBS stammar. Ett viktigt fynd är att antiserum mot protein Rib eller a skyddar möss mot dödlig infektion orsakad av GBS stammar som har motsvarande protein. Detta fynd tyder på att Rib och alfa skulle kunna användas för utveckling av vaccin mot GBS. I arbete II använde vi protein Rib eller a för att vaccinera möss, som sedan fick en dödlig dos av GBS. Möss immuniserade med protein Rib blev inte sjuka av GBS stammar, som uttryckte detta protein, och vaccination med alfa skyddade mot infektion med GBS-stammar, som hade detta protein. Det är således möjligt att en kombination av dessa båda proteiner i ett vaccin skulle kunna skydda mot den största delen av de livshotande GBS-infektionerna hos nyfödda barn. Proteinerna Rib och a reagerar inte immunologiskt med varandra, men har trots detta stora likheter, vilket visas i arbete III, där den genetiska sekvensen, som kodar för protein Rib beskrivs. En för dessa proteiner speciell egenskap är, att de till mer än två tredjedelar består av repeterade sekvenser.



Studier av GAS. Det har sedan länge varit känt att en del stammar av S. pyogenes uttrycker ett protein, R28, som korsreagerar med vissa typer av GBS. Man har inte studerat dettta protein mer ingående, eftersom man inte har trott att R28 har betydelse för bakteriens sjukdomsframkallande förmåga. I arbete IV beskrivs hur vi isolerade, renade och bestämde sekvensen för protein R28. Stora strukturella och andra likheter finns mellan R28 och de ovan beskrivna proteinerna Rib och alfa från GBS. Dessa tre proteiner tillhör således samma proteinfamilj. Två fynd tyder på att R28 har betydelse för den sjukdomsalstrande förmåga hos S. pyogenes. För det första gör R28 det möjligt för bakterien att vidhäfta till humana vaginala celler. För det andra överlever möss, som fått antikroppar mot R28, en dödlig dos av R28-uttryckande bakterier. R28 proteinet kan således framkalla skyddande antikroppar. S. pyogenes, särskilt av typ R28, har rapporterats vara orsak till barnsängsfeber, en sjukdom som många kvinnor dog av i epidemier under 17- och 18-hundratalen, och som fortfarande förekommer i sporadiska fall. Epidemiologiska data och våra molekylära analyser tyder på att R28 proteinet kan ha spelat en central roll vid dessa infektioner. I detta sammanhang är det av speciellt intresse att vi visat att R28 tillåter bakterierna att fästa till epitelceller som kommer från livmoderhalsen.



Korsprotektion mellan GBS och GAS. I arbete V gör vi en närmare jämförelse mellan protein Rib från GBS och R28-proteinet från GAS. Dessa proteiner uppvisar mycket stora biokemiska likheter, men deras immunologiska likhet är mycket begränsad. Trots denna immunologiska olikhet kan vaccination med det ena proteinet ge skydd mot infektion av bakterier med det korsreagerande proteinet. Detta fynd utgör ett intressant exempel på kors-protektion mellan två bakterier som vanligen orsakar mycket olika sjukdomar hos människa. (Less)
