Staphylococcus aureus bacteremia. Serological findings, phenotypic and genotypic characteristics of S. aureus strains.

Abstract
The diagnosis of S. aureus infections is most often based on culture of the organism from various body sites. However, cultures could be false negative due to e.g. previous antibiotic therapy, and in some situations cultures are difficult to obtain. Antibody assays against various S. aureus antigens have been proposed as a way to diagnose S. aureus bacteremia, as well as to distinguish between uncomplicated and complicated S. aureus bacteremia, including S. aureus endocarditis. The diagnosis of S. aureus infections is most often based on culture of the organism from various body sites. However, cultures could be false negative due to e.g. previous antibiotic therapy, and in some situations cultures are difficult to obtain. Antibody assays against various S. aureus antigens have been proposed as a way to diagnose S. aureus bacteremia, as well as to distinguish between uncomplicated and complicated S. aureus bacteremia, including S. aureus endocarditis. Adhesion of bacteria to host proteins is a critical first step in tissue colonization. Collagen is a major component of connective tissue and cartilage. A collagen-binding protein (CnBP) has been described and the corresponding gene, cna, has been sequenced and cloned. Most S. aureus isolates have a capsular polysaccharide (CP) and eleven different serological types have been identified. The aims of these studies was to evaluate if serological assays or combinations of assays can differentiate between uncomplicated and complicated S. aureus bacteremia, S. aureus and non-S. aureus bacteremia or S. aureus and non-S. aureus endocarditis, to examine the distribution of different CP-types and CnBP in S. aureus isolates of various origin and to determine if existence of CnBP and the corresponding cna-gene in S. aureus is a prerequisite for bone and joint infection or endocarditis in patients with S. aureus bacteremia. No serological assays or combinations of assays could differentiate between uncomplicated and complicated S. aureus bacteremia, but combinations of assays could, with high predictive values, discriminate between S. aureus and non-S. aureus endocarditis. As in previous studies, CP 8 was the most predominant CP type in clinical isolates, followed by CP 5, but there was no correlation between CP types and clinical manifestations of S. aureus infections.The cna-gene encodes the only significant collagen-binding protein in S. aureus, and was found in approximately 56% of clinical isolates, but the possession of this gene was equally distributed in clinical isolates of various origin, and is not essential in the pathogenesis of endocarditis or bone and joint infections in humans. S. aureus bakteriemi diagnosticeras oftast genom att man odlar fram bakterier från blodbanan, men har patienten påbörjat antibiotikabehandling innan odlingen utförs, kan detta medföra att bakterierna inte växer ut i blododlingen. Odlingar från inre organ kan vara svåra att erhålla. Redan 1901 föreslog Neisser och Wechsberg att påvisning av antikroppsbildning mot S. aureus beståndsdelar (antigen) hos patienter skulle kunna vara ett sätt att diagnosticera S. aureus infektioner. De senaste 25 åren har ett stort antal antikropps-test (serologier) presenterats. Oftast hat testerna varit inriktade på att särskilja patienter med komplicerad bakteriemi eller hjärtklaffs-infektion (endokardit) från de med okomplicerad S. aureus bakteriemi. Dock har inget enskilt test kunnat särskilja alla patienter med okomplicerad S. aureus bakteriemi från de med komplicerad bakteriemi eller endokardit, det har föreslagits att man skulle kunna använda kombinationer av olika tester. Vilken eller vilka kombinationer av tester som är bäst lämpade för detta har inte studerats. Antikroppar mot olika S. aureus antigen förekommer normalt hos friska personer. Man har därför fått fastställa en övre normalvärdesgräns för varje enskild serologi och/eller undersöka om antikropps-nivån är högre efter en tids sjukdom än den var vid insjuknandet (titer-stegring). Man vet inte varför vissa patienter får allvarliga komplikationer till S. aureus bakteriemi, oftast inte ens om detta beror på faktorer hos värden eller är betingat av egenskaper hos den S. aureus stam som orsakar infektionen. De flesta S. aureus stammar bildar en kolhydrat kapsel och 11 olika kapseltyper är kända. Kapseltyp 5 och 8 är vanligast förekommande hos de stammar som odlas fram från patienter, men om fördelningen av kapseltyper vid olika typer av infektioner har varit ofullständigt utredd. Kollagen är en väsentlig komponent i bindväv, ben och skelett. En del S. aureus stammar bär ett kollagen-bindande protein (CnBP). Utifrån studier på djurmodeller och undersökningar av mindre patientmaterial har det föreslagits att förmåga att binda kollagen är en förutsättning för att S. aureus ska kunna orsaka infektioner i leder och skelett.



Målsättningen med detta avhandlingsarbete har varit:



· att beskriva antikroppssvaret mot kapseltyp 5 och 8 samt mot CnBP hos patienter med S. aureus baktereiemi och kontroller.



· att utvärdera om serologiska tester kan skilja mellan komplicerad och okomplicerad S. aureus bakteriemi, mellan S. aureus endokardit och endokardit av annan etiologi eller mellan S. aureus bakteriemi och bakteriemi orsakad av andra bakterier.



· att undersöka fördelningen av olika kapseltyper samt förekomsten av CnBP hos S. aureus stammar av olika ursprung.



· att fastställa om det är en förutsättning att S. aureus stammar bär på CnBP och den bakomliggande cna-genen för att de ska kunna orsaka infektioner i leder, skelett och hjärtklaffar.



Med hjälp av s.k. ELISA-teknik studerades antikroppssvaret mot olika S. aureus antigen hos patienter med S. aureus endokardit, komplicerad respektive okomplicerad S. aureus bakteriemi, endokardit respektive bakteriemi orsakad av andra bakterier och patienter med feber utan bakteriemi (febrila kontroller). S. aureus stammar från patienter med endokardit, komplicerad respektive okomplicerad bakteriemi, sårinfektioner samt från friska personer som bar bakterien i näsan undersöktes avseende kapseltyp, förmåga att binda kollagen samt förekomst av cna-genen. Kapseltypen bestämdes med hjälp av monoklonala antikroppar mot CP 5 och 8. Den kollagen-bindande förmågan hos S. aureus stammar undersöktes med två tekniker, dels med en partikel-agglutinations assay (PAA), dels med radio-jod märkt kollagen ( 125 I-Cn). Förekomst av cna-genen studerades med hjälp av polymerase chain reaction (PCR).



Antikropps-nivår mot CnBP överstigande gränsvärdet hittades hos 60% patienterna med S. aureus bakteriemi, men testet kunde skilja patienter med S. aureus endokardit eller komplicerad bakteriemi från de med från de med okomplicerad bakteriemi (Delarbete IV). I ett mindre patientmaterial föreföll höga nivåer av antikroppar mot CP 5 och 8 förekomma företrädesvis hos patienter med S. aureus endokardit (Delarbete I), men när detta undersöktes i ett större patientmaterial hittades antikropps-nivåer över gränsvärdet hos endast 27% av patienterna med S. aureus bakteriemi, men det var inte användbart för att särskilja patienter endokardit från övriga patienter med S. aureus bakteriemi (Delarbete II-III).



Kombinationer av serologiska test utvärderades i två studier (Delarbete II-III). Delarbete III är sannolikt den största studie avseende S. aureus serologi som utförts, både vad gäller antalet patienter med S. aureus bakteriemi och antalet serologiska test. Vare sig något enskilt test eller någon kombination av tester kunde skilja patienter med S. aureus endokardit eller komplicerad bakteriemi från patienter medokomplicerad bakteriemi. Däremot kunde en kombination av tester skilja patienter med S. aureus endokardit från patienter med endokardit orsakad av andra bakterier, något som kan vara av värde hos patienter med endokardit, men negativ blododling till följd av antibiotikabehandling.



Kapseltyp 8 var den vanligast förekommande kapseltypen, följt av kapseltyp 5, men fördelningen av kapseltyper var densamma i olika patientgrupper.



Positivt utfall i 125 I-Cn testen korrelerade väl med påvisande av cna-genen, vilket däremot inte PAA gjorde, och denna metod kan inte rekommenderas för att studera den kollagen-bindande förmågan hos S. aureus. Cna-genen fanns hos 56% av S. aureus stammarna, den förekom lika ofta i alla patientgrupperna. Förekomst av cna-genen är inte en förutsättning för att S. aureus ska kunna orsaka infektioner hjärtklaffar, leder eller skelett. (Less)
