Modern inhalation agents and effects of anaesthesia equipment during low-flow anaesthesia

Abstract
Volatile agents are economically and ecologically acceptable only when administered via low-flow systems. However, sevoflurane degrades during low-flow anaesthesia to compound A, and a high carbon dioxide absorber temperature increases this degradation. This thesis suggests that there is a correlation between apparatus dead-space volume and absorber temperature during low- and minimal-flow sevoflurane anaesthesia. Increasing the dead-space volume reduces absorber temperature during low- and minimal-flow sevoflurane anaesthesia.

However, sevoflurane degrades during low-flow anaesthesia to compound A, and a high carbon dioxide absorber temperature increases this degradation. This thesis suggests that there is a correlation between apparatus dead-space volume and absorber temperature during low- and minimal-flow sevoflurane anaesthesia. The main disadvantage of low-flow techniques is that inspired (In) and end-tidal (Et) anaesthetic agent concentrations are not directly related to the vaporiser setting. In the present studies, with desflurane and sevoflurane, there was a significant difference between Et and In concentrations at fresh gas flows (FGFs) of 1.0 and 2.0 l/min. However, the ratio of Et to In concentration remained fairly constant.



Excessive respiratory heat loss may lower body temperature. Artificial humidification of dry inspired gases reduces loss of body heat during anaesthesia, hence the popularity of heat and moisture exchangers (HMEs). In this study, HMEs improved the humidity of the anaesthetic gases at different FGFs, but did not improve maintenance of body temperature during low-flow anaesthesia in adults undergoing elective general or urologic surgery. Lågflödesanestesi (reducerat färskgasflöde till 1,0 l/min) har fördelar genom att minska gaskostnader, reducera gasutsläpp till atmosfären samt öka befuktningen i andningsvägarna. Anestesiologin har under 1990-talet tillförts två nya inhalationsmedel, desfluran och sevofluran. Dessa båda nya inhalationsmedel skiljer sig från de tidigare främst genom sin lägre löslighet i blod vilket ökar inhalationsmedlets styrbarhet och därmed lämpligheten i lågflödesanestesi. Sevofluran interagerar tyvärr med innehållet i koldioxidabsorbern (sodalime) och det kan bildas toxiska metaboliter (compound A) som vid vissa nivåer har visat sig ge njurskador i djurförsök. Eftersom tidigare studier påvisar ett samband emellan koldioxidabsorberns temperatur och bildandet av compound A var syftet i delarbete I att utvärdera om större tidalvolymer med hjälp av en variabel dead-spaceslang, utan att påverka den endtidala koldioxid- koncentrationen, kunde påverka temperaturen i koldioxidabsorbern.



En nackdel med låg- och minimalflödesanestesi (reducerat färskgasflöde till 0,5 l/min) är att den alveolära koncentrationen inte är densamma som inställd koncentration på förgasaren. För att bättre förstå farmakokinetiken vid desfluran- och sevoflurananestesier i ett lågflödessystem ville vi undersöka gaskoncentrationerna i förhållande till förgasarinställningen i arbete II-III.



I delarbete IV utvärderades kroppstemperatur samt absolut och relativ fuktighet i andningsluften, med och utan värmefuktväxlare i kombination med lågflödesanestesi. Eftersom tidigare studier påvisar både negativa och positiva resultat vad beträffar värmefuktväxlarnas förmåga att bibehålla kroppstemperaturen under operation på vuxna patienter, var detta ett andra delmål i arbete IV. Sammanfattningsvis har delarbete I givit oss kunskap om ett enkelt sätt att reducera temperaturen i koldioxidabsorbern genom tillägg av ett ökat apparatdead-space. Absorbertemperaturen reducerades ifrån 44,8 till 37,5 °C vid ett färskgasflöde på 1,0 l/min.



Farmakokinetiken hos desfluran och sevofluran i vårt lågflödessystem har samma karaktäristika som i andra system för lågflödesanestesi av typ bag-in-bottle. Vid ett färskgasflöde på 1,0 l/min fanns signifikanta skillnader mellan inspiratoriska och alveolära koncentrationer. Delarbete II visade att desfluranets låga löslighetskoefficient medgav att förgasarinställningen, efter en denitrogeneringsperiod på 5 minuter med 4,5 liters färskgasflöde, kunde bibehållas oförändrad vid en anestesi tid upp till 2 timmar. I delarbete III användes sevofluran med samma denitrogeneringsperiod och med samma färskgasflöde. Resultaten indikerar att, förgasarinställningen med sevofluran troligtvis måste korrigeras efter denitrogeneringsfasen för att anestesin inte ska bli för ytlig.



I delarbete IV ökade klimatiseringen av anestesigaserna vid användande av värmefuktväxlare upp till färskgasflöden på 6 l/min. Dock sågs ingen påverkan på kroppstemperaturen under en tvåtimmars anestesi, även om värme-fuktväxlaren kombinerades med lågflödesanestesi. (Less)
