LYME BORRELIOSIS Detection and identification of the infecting genotype

Abstract
Lyme borreliosis is caused by the spirochete Borrelia burgdorferi sensu lato, a bacterium with a parasitic life-style, which is transmitted between hosts by the Ixodes tick-vector. Humans will incidentally become infected. The resulting illness can involve the skin, nervous system, joints and heart. Southern Sweden has been identified as an area endemic for Lyme borreliosis. The most common clinical manifestations are erythema migrans and neuroborreliosis. Lyme borreliosis is caused by the spirochete Borrelia burgdorferi sensu lato, a bacterium with a parasitic life-style, which is transmitted between hosts by the Ixodes tick-vector. Humans will incidentally become infected. The resulting illness can involve the skin, nervous system, joints and heart. Southern Sweden has been identified as an area endemic for Lyme borreliosis. The most common clinical manifestations are erythema migrans and neuroborreliosis. Three major species have been associated with Lyme borreliosis in Europe, B. burgdorferi sensu stricto (s. s.), B. afzelii and B. garinii. The presence of the two latter species was indicated in a seroepidemiological study where a Lyme borreliosis infected population, living in southern Sweden, showed preferential seroreactivity to the species-specific loop of the surface membrane protein P66 originating from B. afzelii and B. garinii. In parallel, a study was performed using culture-based analytical method. Subsequently determination of the OspA serotype (protein-based), and the 16S rDNA genotype, indicated that, B. afzelii (OspA serotype 2) and B. garinii (OspA serotype 5 and 6) were present in skin and cerebrospinal fluid from patients in the region. Using a direct molecular method, that combines nested OspA PCR and sequence analysis, the third major species, B. burgdorferi s. s., was identified in a clinical specimen for the first time in Sweden. In order to further improve the sensitivity and quantification of Borrelia spirochetes, we designed a RT real-time PCR assay directed against 16S rRNA, a method providing both excellent sensitivity and reproducibility. In conclusion, the three major Lyme Borrelia species, B. burgdorferi s. s., B. afzelii and B. garinii, have been found to be associated with Lyme borreliosis in southern Sweden. (Less)

Hudinfektionen har ett karakteristiskt utseende i form av en rundad förändring omgiven av en rodnad ytterkant med bleknad mitt. Erythema migrans har en god möjlighet att läka spontant men ska behandlas med antibiotika för att förhindra att infektionen sprider sig i kroppen. Bakterien kan sprida sig till centrala nervsystemet och orsaka en lindrigare form av hjärnhinneinflammation som inte så sällan är kombinerad med en ansiktsförlamning och nervrots smärtor. Infektionen kan också engagera leder och hjärta. Borrelios diagnostiseras med hjälp av den kliniska bilden (symptom) och patientens sjukhistoria (fästingbett) tillsammans med fynd av antikroppar mot Borrelia. Borrelia bakterien och Ixodes fästingar finns över hela norra halvklotet. Det finns åtminstone 11 olika Borrelia arter och 3 av dessa ger sjukdom hos människa, B. burgdorferi (sensu stricto), B. afzelii och B. garinii. Tidigare studier har visat att uppemot 20% av fästingarna i södra Sverige bär på bakterien. Dessutom har B. afzelii och B. garinii odlats fram från patienter med borrelios i Stockholmstrakten. De aktuella studierna syftade till att kartlägga den kliniska bilden av borrelios samt att påvisa och typa Borrelia bakterier som ger sjukdom hos människa i södra Sverige. I den inledande studien studerades borrelios ur ett kliniskt epidemiologiskt perspektiv och vi fann att hela södra Sverige utgör ett endemiskt område för infektionen. Hudinfektion, erythema migrans, följt av infektion i centrala nervsystemet, neuroborrelios, är de vanligaste sjukdomsformerna. En grupp patienter med fastställd borrelios i södra Sverige visade sig ha antikroppar mot ett ytprotein (P66) härrörande från B. afzelii och B. garinii, ett indirekt bevis för att dessa arter är orsak till infektionen i området. Dessutom lyckades vi isolera de båda arterna från patienter med erythema migrans och neuroborrelios bosatta i området i en studie baserad på en odlingsmetod. Ytterliggare information om de Borrelia arter som ger infektion hos människa i södra Sverige fick vi genom att använda en metod som påvisar genfragment (DNA) från bakterien i patientprov. Med denna teknik kunde vi visa att också den tredje arten B. burgdorferi sensu stricto orsakar sjukdom i södra Sverige. Resultaten från våra delstudier talar för att borrelios är en vanlig fästingburen infektionssjukdom i södra Sverige. Infektionen orsakas av de 3 arterna B. burgdorferi (sensu stricto), B. afzelii och B. garinii. Tillgång till de Borrelia stammar som vi isolerat kommer att ha betydelse för fortsatta studier samt utveckling av diagnostik och vaccin. (Less)
