Molecular studies of genetic changes in myxoid and round cell liposarcoma

Abstract
Chromosomal translocations commonly result in the production of fusion genes and the fusion genes are often tumor-type specific. In myxoid and round cell liposarcomas (MLS/RCLS), almost 95% of the cases carry a t(12;16)(q13;p11). In the remaining 5% of the MLS/RCLS tumors, another translocation and fusion gene can be found, i.e. the t(12;22)(q13;q12). These translocations fuse CHOP on chromosome 12 to either TLS on chromosome 16 or EWSR1 on chromosome 22. The result is a TLS-CHOP or EWS-CHOP fusion protein that is believed to be a key player in the development of MLS/RCLS.

In the remaining 5% of the MLS/RCLS tumors, another translocation and fusion gene can be found, i.e. the t(12;22)(q13;q12). These translocations fuse CHOP on chromosome 12 to either TLS on chromosome 16 or EWSR1 on chromosome 22. The result is a TLS-CHOP or EWS-CHOP fusion protein that is believed to be a key player in the development of MLS/RCLS.



Normally, CHOP plays a fundamental role in adipogenesis by forming heterodimers with members of the C/EBP family of transcription factors. TLS-CHOP is capable of dimerizing with the same proteins as CHOP, but the outcome is radically different. This thesis focuses on TLS-CHOP and the hypothesis that TLS-CHOP may function as an aberrant transcription factor. As such TLS-CHOP is believed to regulate the expression of a cascade of downstream genes that contribute to the phenotype and malignancy of MLS/RCLS.



We have found that the TLS-CHOP fusion protein localizes to distinct nuclear structures which differs largely from the normal TLS and CHOP patterns. The nuclear structures do not overlap with PML nuclear bodies and furthermore, PML nuclear bodies are dislocated in TLS-CHOP-expressing cells. The abnormal localization of the PML tumor suppressor may be an important part of the transforming process in these tumors.



Using a subtraction hybridization technique, we have found that several genes are aberrantly expressed in MLS/RCLS compared to normal adipose tissue and may have a function in the development of these tumors. This work resulted in the identification of a novel gene which seems to be associated with a low degree of differentiation. At present the normal function of this protein, LEPREL1, is not known. However, its different domains and intracellular distribution points in the direction that this protein functions as an enzyme in the ER and Golgi pathway.



One of the genes identified as over-expressed in MLS/RCLS was the RET proto-oncogene. Further studies showed that the tumor cells produce the activating ligand persephin and that the receptor is phosphorylated. This indicates that it is activated and capable of signaling proliferation and anti-apoptosis into the cell nucleus. Thus, there seems to be an autocrine growth stimulatory loop running in MLS/RCLS tumors and disrupting the RET signaling pathway may be an attractive target for therapy in MLS/RCLS. (Less)

En typ av mutation som kan uppstå är en sk kromosomtranslokation. Med det menas att två (eller ibland flera) kromosomer har gått sönder och sedan "lagats" på fel sätt så att en blandkromosom uppstått med bitar från båda ursprungskromosomerna. Sådana kromosomtranslokationer kan man hitta i många tumörtyper och genom att studera tumörcellernas DNA så har man kunnat påvisa ett samband mellan vissa typer av translokationer och specifika cancerformer. En sådan tumörtypsspecifik kromo-som-translokation finns t ex i ca 95% av fallen i en cancerform som heter myxoid och rundcells liposarkom (MLS/RCLS). I MLS/RCLS har kromosom 12 och kromo-som 16 (alternativt kromosom 22 i några procent av fallen) gått sönder och satts ihop till en blandkromosom.



På DNA-nivå har det visat sig att två gener ligger precis i brottspunkterna och fogas samman i den nya blandkromosomen. De gener som berörs är CHOP på kromosom 12 och TLS på kromosom 16 (eller ibland närbesläktade EWS på kromosom 22). CHOP är en transkriptionsfaktor som påverkar många andra gener så att de aktiveras och producerar protein. I vanliga fall ska inte CHOP proteinet finnas i cellen utan behövs endast då cellen utsätts för olika typer av stress eller då cellen ska omvandlas till en fettcell. Då aktiveras CHOP genen och det bildas CHOP protein som hindrar cellen från att växa. TLS däremot är ett sk "houskeeping" protein vilket betyder att TLS behövs hela tiden för att cellen ska kunna fungera normalt. I MLS/RCLS så har början på TLS fogats samman med hela CHOP och det nya TLS-CHOP proteinet produceras hela tiden i cellen. Eftersom TLS-CHOP (eller den besläktade EWS-CHOP) finns i i så gott som samtliga fall av MLS/RCLS och inga andra tumörtyper har hittats som innehåller TLS-CHOP proteinet så är det rimligt att anta att TLS-CHOP spelar en viktig roll i uppkomsten av MLS/RCLS.



I första arbetet visade vi att det nya TLS-CHOP proteinet inte återfinns på samma plats i cellerna som vare sig TLS eller CHOP, utan ansamlas i distinkta aggregat inne i cellkärnorna. Dessa aggregat liknar sk "PML nukleära kroppar" som tidigare beskrivits. Vår första studie visade dock att TLS-CHOP aggregaten inte är samma som dessa nukleära kroppar och att TLS-CHOP dessutom påverkar distribueringen av PML nukleära kropparna i cellerna. Eftersom PML är ett sk tumörsuppressor protein, som på ett väldigt förenklat sätt kan sägas förhindra tumörbildning, så kan den TLS-CHOP inducerade feldistribueringen av PML bidra till den oncogena effekten som TLS-CHOP har.



I andra arbetet försökte vi hitta gener som är överaktiva i MLS/RCLS i jämförelse med hur samma gener fungerar i normal fettvävnad. Genom att identifiera och karaktärisera sådana gener hoppas vi kunna öka förståelsen för hur TLS-CHOP fungerar i cellen och bidra till att viktiga pusselbitar i kunskapen kring tumörbilden faller på plats. I detta arbetet lyckades vi identifiera fem sådana överaktiva gener som vi kom att kalla MLATar (står för Myxoid Liposarcoma Associated Transcript).



I tredje arbetet valde vi att försöka ta reda på mer om en av de gener som identifierades i andra arbetet. Ingen annan hade tidigare beskrivit denna DNA sekvens och vi kunde i detta arbetet visa att den nya genen är överaktiv i flera tumörtyper inkluderande MLS/RCLS samt i en sorts stamceller som heter mesenkymala stamceller. I detta arbetet tittade vi även på hur proteinet som bildas från genen ser ut och försökte dra slutsatser om hur proteinet fungerar genom att jämföra med andra proteiner som det har likheter med. Baserat på dessa likheter valde vi att döpa genen till LEPREL1 (Leprecan-like 1). Genom att märka proteinet med en grön signal så kunde vi även studera hur proteinet distribuerades inne i cellerna. Ytterligare studier kring LEPREL1 behövs för att få klarhet i vilken roll den har i utvecklingen av MLS/RCLS.



I fjärde arbetet studerade vi ytterligare en av de gener vi identifierade i andra arbetet. Denna gen, RET (eller MLAT6), är en känd proto-oncogen d v s förändringar i denna ger upphov till en oncogen som i sin tur har visat sig bidra till tumörbildning i många olika celltyper. Vi letade efter sådana förändringar, mutationer, i genen för att kunna styrka att RET fungerar som en oncogen i MLS/RCLS. Vi kunde dock inte hitta några mutationer. Det finns emellertid en annan möjlighet för proto-oncogenen att bli en oncogen. I normala fall aktiveras genen av en liten molekyl som kallas ligand. Om en sådan aktiveringsmolekyl finns närvarande så kommer den att aktivera proto-oncogenen på liknande sätt som en mutation skulle kunna göra. Vi valde därför att leta efter sådana ligander och fann att tumörerna faktiskt själva producerar denna aktiveringsmolekyl (persephin) och på så sätt konstant kan aktivera RET.



Reslutaten i denna avhandling bidrar till att öka förståelsen för hur TLS-CHOP påverkar cellerna och hur detta kan bidra till tumörutvecklingen. Fyndet att RET signalvägen är aktiverad i MLS/RCLS pekar på möjligheten att behandla dessa tumörer genom att blockera signalen. Dessutom leder identifieringen av den nya genen, LEPREL1, till ökad kunskap om genomet och dess olika funktioner. (Less)
