Regulation of inducible nitric oxide synthase - Consequences in experimental models of bladder disease

Abstract
Inducible nitric oxide synthase (iNOS) expression is associated with various pathophysiological conditions in the lower urinary tract. The aims of this thesis have been to investigate the regulation of iNOS in bladder smooth muscle cells and the consequences on neuromuscular regulation, cell growth and differentiation. Primary cultures of rat bladder smooth muscle cells (BSMC) expressed iNOS mRNA and protein upon stimulation with the cytokines IL-1β and TNF-alpha in combination. No constitutive isoforms of NOS were detected at the mRNA level. BSMC were put into different stages of differentiation. Inducible nitric oxide synthase (iNOS) expression is associated with various pathophysiological conditions in the lower urinary tract. The aims of this thesis have been to investigate the regulation of iNOS in bladder smooth muscle cells and the consequences on neuromuscular regulation, cell growth and differentiation. Primary cultures of rat bladder smooth muscle cells (BSMC) expressed iNOS mRNA and protein upon stimulation with the cytokines IL-1β and TNF-alpha in combination. No constitutive isoforms of NOS were detected at the mRNA level. BSMC were put into different stages of differentiation. The capacity of BSMC to express iNOS was negatively correlated to differentiation status measured as smooth muscle myosin-heavy chain expression. iNOS was shown to be preferentially expressed in immature dedifferentiated BSMC. Actin cytoskeletal dynamics and Rho signalling were involved in regulation of cytokine-induced iNOS expression in BSMC. Phenotypic changes and impairment in actin cytoskeletal formation may potentiate cytokine activation and in turn increase NO production during bladder disease. The consequences of iNOS induction on bladder neuromuscular function were investigated by incubating isolated rat bladder strips with cytokines and recording the mechanical activity in organ baths. iNOS protein was observed in the smooth muscle layer in cytokine stimulated bladder strips. Nerve-mediated, but not acetylcholine-mediated, contraction of the bladder was impaired in bladder strips exposed to cytokines. This impairment was restored by the iNOS inhibitor aminoguanidine and partially by the neurotrophic factor BDNF. iNOS induction affected bladder function and bladder nerves were more sensitive to NO exposure than smooth muscle. The activity and expression of different NOS isoforms were investigated in an in vivo model of bladder hypertrophy induced by urethral obstruction in rats. During obstruction the nNOS activity decreased whereas iNOS activity increased initially. The total NOS activity was decreased. iNOS was expressed in smooth muscle, urothelium and inflammatory cells. Exogenous NO exposure decreased DNA- and protein-synthesis and stimulated differentiation in cultured BSMC. The results indicate that the decreased NO production during obstruction may stimulate a growth response and phenotypic changes in the bladders smooth muscle. The results in this thesis show that the smooth muscle in the bladder should be considered as a source of iNOS expression and NO production during inflammation. Neuronally derived NO is suggested to be involved in bladder smooth muscle physiology by maintaining and stabilizing the smooth muscle phenotype. (Less)

NO är idag en väl etablerad mediator i nedre urinvägarna och dess fysiologiska betydelse har studerats intensivt. Mycket tyder på att NO frisatt från nerver av enzymet nNOS är involverat i relaxation av utflödesregionen vid blåstömning. Däremot finns få data som stöder att neuronalt NO reglerar blåsans relaxation vid fyllnad. iNOS bildar mycket höga lokala nivåer av NO vilket kan vara skadligt för kringliggande vävnad genom att NO bildar reaktiva radikaler som i sin tur kan reagera med proteiner och förändra proteiners struktur och därmed funktion. NO medierad vävnadsskada av muskulatur, nerver och urotelceller i nedre urinvägarna med eventuell påföljande funktionsrubbningar har hittills inte uppmärksammats. Ett flertal funktionsrubbningar i nedre urinvägarna skulle teoretiskt sätt kunna involvera NO-medierad vävnadsskada. Den inducerbara isoformen av NOS har detekterats i urinvägarna, framförallt i urotelet och i inflammatoriska celler, vid urinvägsinfektion medan iNOS responsen i glatt muskulatur inte tidigare undersökts. Syftet med avhandlingen har varit att undersöka regleringen av iNOS i blåsans glatta muskulatur och eventuella konsekvenser av en iNOS uppreglering för neuromuskulär funktion, celltillväxt och differentiering. Glatta muskelceller isolerades från råttblåsa och studerades i cellkultur. Ostimulerade celler uttryckte ej iNOS. Blåsans glatta muskelceller uttryckte iNOS mRNA och protein vid stimulering med cytokinerna IL-1β i kombination med TNF-alpha. Inga konstitutiva isoformer av NOS d v s eNOS eller nNOS detekterades på mRNA nivå. Eftersom glattmuskulatur är den dominerande vävnadstypen i blåsan är det intressant att denna celltyp kan producera NO vid inflammation. Ökad tillgång på enzymsubstratet L-arginin ledde till ökad NO produktion. Glatta muskelceller som har en låg grad av differentiering, vilket är att förvänta vid t ex skada och hypertrofi (celltillväxt), visades ha större benägenhet att uttrycka iNOS än fullt differentierade muskelceller. Genom att odla glattmuskelceller på laminin i serumfritt medium tillsammans med tillväxtfaktorn IGF-1 uttrycktes differentieringsmarkören SM-MHC (smooth muscle-myosin heavy chain) men inget iNOS vid cytokinstimulering. Glatta muskelceller som försattes i ett mer dedifferentierad tillstånd genom att odlas på plast eller exponering för mitogena faktorer uttryckte iNOS men mindre SM-MHC. Fynden att en dedifferentierad cellfenotyp gynnar iNOS uttryck bekräftades även då cellernas grad av differentiering samt iNOS uttryck påverkades genom att interagera med Rho-signallering och aktincytoskelett. Resultaten indikerar att fenotypiska förändringar och aktinpåverkan kan potentiera iNOS svaret och därmed öka NO-produktionen vid olika blåsfunktionsstörningar. Konsekvenserna av cytokinstimulering med påföljande iNOS induktion i blåsan undersöktes genom att hela muskelstrips från råttblåsa inkuberades med cytokiner varefter dess neuromuskulära funktion undersöktes i organbad. Nervmedierad, men ej acetylkolinmedierad, kontraktion av blåsan försämrades i blåsstrips som stimulerats med cytokiner. iNOS protein observerades i muskelcellagret hos cytokinstimulerade blåsstrips. Tillsats av iNOS hämmaren aminoguanidine medförde att nervfunktionen återhämtades helt vilket tyder på en involvering av NO. Neurotrofinet BDNF visades också ha en skyddande effekt mot cytokin-medierad nervpåverkan. Dessa studier visar att iNOS induktion påverkar blåsans funktion och att blåsans nerver är mer känsliga för NO exponering än den glatta muskulaturen. Aktiviteten och uttrycket av olika NOS isoformer undersöktes i en patofysiologisk in vivo modell på råtta där blåshypertrofi framkallats genom uretraobstruktion. Urinblåsan svarar på obstruktion genom att genomgå en tillväxt som leder till muskelhypertrofi. Vid obstruktion upptäcktes att NOS aktiviteten ändrats. nNOS aktiviteten minskade och iNOS aktiviteten ökade initialt men totalt sett minskade NOS-aktiviteten vid obstruktion. iNOS uttrycktes i blåsans glatta muskulatur samt i urotel och inflammatoriska celler. Exogen NO exponering minskade DNA- och proteinsyntes samt stimulerade differentiering av glatta muskelceller. Resultaten tyder på att blåsans NO produktion minskar vid obstruktion och att minskad NO produktion kan stimulera tillväxtprocesser och fenotypiska förändringar i blåsans glatta muskulatur. Sammanfattningsvis visar denna avhandling att den glatta muskulaturen i blåsan kan uttrycka iNOS och bilda NO vid inflammation. Dedifferentierade glatta muskelcellerna visades vara mer benägna att uttrycka iNOS än differentierade celler vilket kan innebära en potentierad iNOS/NO produktion vid patologiska förändringar i urinblåsan. Induktion av iNOS, till följd av cytokinexponering, medförde förändringar i blåsans funktion och nerver var mer känsliga än den glatta muskulaturen. Vidare tyder resultaten på att NO påverkar fenotyp och celltillväxt av glatta muskelceller i blåsan vilket kan ha betydelse för dess fysiologi och integritet. En rubbning i NO systemet skulle således kunna leda till förändringar i blåsans struktur och därmed potentiellt dess funktion. (Less)
