Febrile Urinary Tract Infection: P fimbriae, Innate Host Response and Bacteremia

Abstract
Urinary tract infections (UTI) are among the most common human infections, and febrile UTI with bacteremia the most severe form. The relation between bacteria and host decides disease severity. The dominating uropathogen is Escherichia coli (E. coli). P fimbriae are the virulence factor of E. coli most clearly associated to severe disease. P fimbriated E. coli predominated in bacteremic febrile UTI in both women and men, and carried papDNA sequences of papGIA2 and prsGJ96 adhesin type in both bacteremic and nonbacteremic patients. papGIA2 + E. coli were the major uropathogens in women of all ages, and the major cause of bacteremia in women. prsGJ96 + E. coli were common uropathogens in men, and associated with urinary tract abnormalities.... Urinary tract infections (UTI) are among the most common human infections, and febrile UTI with bacteremia the most severe form. The relation between bacteria and host decides disease severity. The dominating uropathogen is Escherichia coli (E. coli). P fimbriae are the virulence factor of E. coli most clearly associated to severe disease. P fimbriated E. coli predominated in bacteremic febrile UTI in both women and men, and carried papDNA sequences of papGIA2 and prsGJ96 adhesin type in both bacteremic and nonbacteremic patients. papGIA2 + E. coli were the major uropathogens in women of all ages, and the major cause of bacteremia in women. prsGJ96 + E. coli were common uropathogens in men, and associated with urinary tract abnormalities. papGIA2+ isolates was the only cause of bacteremia in healthy patients. Bacteremia in patients with host compromise was caused by a mixture of papGIA2+, prsGJ96+ or pap negative isolates. The results emphasise the strong influence of host factors on the selection of uropathogenic E. coli. Febrile UTI was accompanied by a broad and diverse local chemokine response with both CXC and CC chemokines, and by both local and systemic IL-6 responses, as measured in urine and serum. The responses showed therapy-related kinetics. The response magnitudes were associated to bacteremia and disease severity, and influenced by gender and general health. papGIA2+ E. coli were associated to pyelonephritis symptoms, high incidence of bacteremia, and high fever, CRP and higher CCL2/MCP-1 response. Febrile UTI patients with prsGJ96+ isolates had instead more focal symptoms from the lower urinary tract. Systemic IL-6 responses occurred regardless of bacteremia, suggesting that locally produced IL-6 trigger systemic responses. Urine CXCL8/IL-8 was found in all patients and may be used as a marker for UTI, and urine CCL2/MCP-1 for disease severity, correlating to kidney involvement and bacteremia. The results characterise papGIA2+ E. coli as the most virulent strains, with invasive abilities and causing bacteremia, kidney involvement and intense host responses. Seven days of adequate antibiotic therapy was shown to be effective and safe in hospitalised patients with community-acquired febrile UTI, regardless of gender, compromising conditions or bacteremia, and with no difference in clinical and bacteriological outcome compared to 14 days of therapy. Patients with persistence of the original uropathogen developed early clinical infection, whereas all super- or reinfections with gram-positive bacteria were asymptomatic. Bacteremic patients with compromising conditions might need closer observations or longer therapy until further studies. Women had a worse secondary outcome, with lower clinical and bacteriological cure rates, which probably reflect the natural course of UTI in women. (Less)

Svårighetsgraden vid UVI varierar beroende på samspelet mellan värdens försvar och bakteriens virulens. En avgörande faktor är bakteriernas förmåga att vidhäfta vid slemhinnor och deras förmåga att invadera till vävnaderna och blodbanan, vilket leder till sepsis. Ett antal olika så kallade virulensfaktorer har identifierats vilka tillsammans bidrar till bakteriernas förmåga att åstadkomma olika typer av UVI. P fimbrier är långa hårliknande utskott på E. colis yta med förmåga att binda till ytmolekyler på humana celler, och är den virulensfaktor hos E. coli som visar det tydligaste sambandet med allvarlig UVI, som njurbäckeninflammation och spridning av bakterierna till blodbanan (bakteriemi). Patienterna har olika mottaglighet för infektion, beroende på det allmänna hälsotillståndet och förekomsten av s. k. komplicerande faktorer som t ex störningar i urinvägarnas funktion eller sjukdomar som påverkar immunförsvaret. Komplicerande faktorer hos patienten innebär att bakterier med lägre grad av virulens kan åstadkomma infektion. Symptombilden och infektionens svårighetsgrad avspeglar också värdens förmåga att svara på de infekterande bakterierna. Vid febril UVI har bakterierna aktiverat både det lokala värd-svaret i urinvägarna och det systemiska svaret med feber, allmänpåverkan och stegring av akuta fas-reaktanter i blodet. Denna avhandling har studerat förekomsten av virulensfaktorer, med fokus på P fimbrier, hos E. coli vid febril UVI och vid bakteriemi, samt det inflammatoriska värd-svaret, och samspelet mellan värd och bakterie. Antibiotikabehandling av febril UVI är en självklarhet, men hur långvarig behandlingen behöver vara är fortfarande delvis oklart. Trenden har gått mot en allt kortare behandlingstid med de nyare och mer effektiva urinvägsantibiotika som finns idag. Rekommendationer för kortade behandlingstider finns för icke-febril UVI och för febril UVI av måttlig eller lättare svårighetsgrad, men ej för patienter med allvarlig febril UVI. Vi har därför studerat detta och i avhandlingen ingår en studie om antibiotikabehandling vid febril UVI.



Studierna visar att de flesta E. coli som orsakar febrila urinvägsinfektioner bär P fimbrier, samt att friska personer enbart får bakteriemi vid febril UVI när de är infekterade med P fimbrierade E. coli. De E. coli bakterier som ej har P fimbrier orsakar bakteriemi endast om patienten har komplicerande faktorer, såsom patologiska tillstånd i urinvägarna eller andra sjukdomar.



Den vidhäftande delen av P fimbrien kallas PapG. Det finns olika genetiska varianter av PapG, och de vanligast förekommande gentyperna hos sjukdomsalstrande E. coli är papGIA2 och prsGJ96. De olika PapG-typerna binder till olika vidhäftningsmolekyler (receptorer) på den humana cellytan. Våra studier visar att papGIA2+ E. coli är den huvudsakliga orsaken till febril UVI hos kvinnor i alla åldrar och till bakteriemi hos kvinnor. prsGJ96+ E. coli är den vanligaste sjukdomsalstraren hos män, och associerad till patologiska tillstånd i urinvägarna. prsGJ96+ E. coli och pap-negativa E. coli orsakar bakteriemi endast hos personer med komplicerande faktorer. Infektion med papGIA2+ E. coli leder oftare till bakteriemi än med de andra typerna av E. coli, oavsett kön eller komplicerande faktorer hos patienten, och är den enda orsaken till bakteriemi hos friska personer. Resultaten visar att värdfaktorer som kön och bakomliggande sjukdomar spelar en stor roll vid urvalet av de sjukdomsalstrande bakterierna och deras möjlighet att orsaka infektion, och att papGIA2+ E. coli är den mest virulenta bakterietypen.



Patienternas svar på infektionen avspeglas i de olika signalsubstanser, s. k. cytokiner, som bildas dels lokalt i urinvägarna och dels systemiskt, Vi har mätt interleukin-6 (IL-6) i blod och i urin, och chemokiner i urin. IL-6 är en cytokin med både inflammationsframkallande och inflammationsdämpande egenskaper och som bl.a. påverkar feber och tillverkningen av akut fas-reaktanten CRP. Chemokiner aktiverar och tillkallar olika försvarsceller, såsom neutrofiler vilka är de vita blodkroppar som avdödar bakterier vid UVI. Patienter med febril UVI har både ett lokalt och ett systemiskt IL-6 svar, och de högsta nivåerna ses hos patienter med bakteremi. IL-6 nivåerna normaliseras efter insatt behandling. Även chemokin-nivåerna i urin är förhöjda vid febril UVI, med korrelation till sjukdomens svårighetsgrad och till bakteriemi, och med påverkan av värdfaktorer som kön och allmän hälsa. Ett flertal olika chemokiner kunde uppmätas i urinen. Detta överensstämmer med experimentella studier av humana slemhinnecellers förmåga att producera signalsubstanser vid infektion och med bakteriernas förmåga att påverka humana celler från urinvägarna. Alla patienterna hade tydligt mätbara halter av CXCL8/IL-8 i urinen vid ankomsten till sjukhus, och flertalet även av de övriga 8 chemokinerna. Koncentrationen av CCL2/MCP-1 korrelerade tydligt med njurengagemang och med bakteriemi. Studierna visar att bestämningar av CXCL8/IL-8 i urin kan användas som allmän markör för UVI, och av CCL2/MCP-1 i urin som markör för svårighetsgrad vid febril UVI.



De papGIA2+ E. coli var de mest virulenta sjukdomsalstrarna och orsakade tydliga tecken på njurbäckeninflammation, högre feber, och högre CRP- och chemokin-nivåer, fr.a. av CCL2/MCP-1, jämfört med de andra typerna av E. coli. Studierna visar att bakteriens virulensförmåga direkt påverkar värdens inflammatoriska svar, samt att sjukdomsmekanismerna vid UVI delvis är olika mellan män och kvinnor.



Studien av behandlingstid visar att 7 dagars behandling med effektiva och adekvata antibiotika har en lika god effekt som 14 dagars behandling vid febril UVI, och att detta gäller oavsett kön, komplicerande sjukdomar eller bakteriemi. Vidare visades att om patienten vid kontroll 7 – 10 dagar efter avslutad behandling hade samma bakterie i urinen som vid ursprungsinsjuknandet, var detta alltid förenat med en pågående eller snart debuterande symptomgivande infektion. Symptomfritt bärarskap av grampositiva bakterier i efterförloppet behövde däremot ej behandlas. Kvinnor hade allmänt en sämre UVI prognos såtillvida att de oftare fick recidiv under uppföljningsmånaderna, men detta återspeglar troligen den normala epidemiologien för UVI hos kvinnor och skiljde sig ej mellan de olika behandlingstiderna. Således är 7 dagars antibiotika en säker behandling vid febril UVI, men vid kombinationen bakteriemi och bakomliggande sjukdomar hos patienten rekommenderas tills vidare noggrann observation och/eller längre behandlingstid i avvaktan på ytterligare studier.
