Abstract
Background: Anterior cruciate ligament injury is a common knee injury and the treatment may be surgical and/or conservative. Different studies have shown surgical and conservative treatment, respectively, as the best intervention. Therefore it is uncertain which treatment is preferable. When searching in PubMed, no articles were found describing the thoughts and experiences of orthopedists and physiotherapists regarding the treatment of individuals with an anterior cruciate ligament injury.

Background: Anterior cruciate ligament injury is a common knee injury and the treatment may be surgical and/or conservative. When searching in PubMed, no articles were found describing the thoughts and experiences of orthopedists and physiotherapists regarding the treatment of individuals with an anterior cruciate ligament injury.

Purpose: The objective of this study was to examine the thoughts and experiences of Swedish and Austrian orthopedists and physiotherapists regarding the management and rehabilitation of an individual with an anterior cruciate ligament injury.

Study design: Qualitative pilot study.
Participants and Method: A total of three orthopedists and six physioterapists participated in this qualitative study. The participants were selected using purposive sampling to ensure a
wide variety. The material was analyzed by utilizing content analysis according to Graneheim & Lundman.

Result: The analysis resulted in three categories: The significance of the first meeting and examination; Experiences of decision making and interventions; Factors of significance for the outcome. The result showed many similarities, but also differences between the countries.
The treatment recommendations differed, particularly in the acute phase. In Austria, acute surgery was primarily recommended and in Sweden, the recommendation was rehabilitation with a subsequent decision about surgery. The experience among the participants was that the
first meeting with the patient was of great significance. The description of the injury gave
important information to determine the diagnosis and Lachmann’s test was the most important test in the clinical examination. All the physiotherapists had the experience that they were free to choose the treatment for the patient within the existing guidelines. The motivation of the patient and also the cooperation between the physiotherapist and the patient were described as the most significant factors for a good outcome.

Conclusion: A mutual experience among the participants was that the choice of treatment hasto be made with consideration to the individual’s situation. Further research is needed, in a larger perspective, to gain knowledge about which treatment is preferable in diverse
situations, and also how the rehabilitation should be structured.
