Ett sätt att fara illa - En tolkande fenomenologisk studie om självskadande sexuella beteenden hos kvinnor i DBT-behandling

Abstract
The purpose of this study was to explore how women in dialectical behavior therapy understand their experiences of engaging in self-harming sexual behavior. Semi-structured interviews were conducted with three participants, who stated that they had engaged in sexual self-harm. Interpretative Phenomenological Analysis (IPA) was used to analyze the material and six main themes were identified. The study illustrates how performing sexual acts can become a way of hurting oneself. Intensive longing for closeness and validation, as well as feelings of low self-worth were described. The purpose of this study was to explore how women in dialectical behavior therapy understand their experiences of engaging in self-harming sexual behavior. Semi-structured interviews were conducted with three participants, who stated that they had engaged in sexual self-harm. Interpretative Phenomenological Analysis (IPA) was used to analyze the material and six main themes were identified. The study illustrates how performing sexual acts can become a way of hurting oneself. Intensive longing for closeness and validation, as well as feelings of low self-worth were described. Taken together, these were elements that seemed to cause high levels of vulnerability. The accounts portrayed a feeling of inevitability of negative consequences associated with engaging in sexual self-harm and was contrasted by a strong hope for an alternative ending that was different from the consequences they always experienced. The informants spoke about fear of separation, which led to attempts, sometimes expressed in self-harming sexual behaviors, to control the separation that was anticipated. The stories contained a complex interplay of variables whereby participants described feeling a responsibility to accept all sexual behaviors, even those that were unwanted, painful and disgusting, if they initiated contact with men. Compared to other self-harming behaviors, sexual self-harm seemed easier to disguise as a healthier behavior, despite the severe suffering it caused. (Less)

Avslutningsvis framträdde sexuell självskada, jämfört med andra självskadebeteenden, som lättare att förklä som ett mer sunt beteende, trots det stora lidande den förorsakade. Informanterna upplevde sig, via större medvetenhet om hur beteendet påverkat dem, vara på väg mot en förändring. (Less)
