Suicidriskbedömningens praktik. En kvalitativ studie.

Studiens syfte var att utforska hur kliniskt verksamma behandlare beskriver suicidriskbedömningens praktik: genomförandet av suicidriskbedömningar, vilka processer och överväganden som vägleder bedömningen och vilka faktorer som påverkar bedömningen. En fokusgrupp med sex deltagare samt sex semistrukturerade individualintervjuer genomfördes, de tolv deltagarna var personer med olika yrken som alla var verksamma inom vuxenpsykiatrisk öppenvård och återkommande genomförde suicidriskbedömningar. Studiens syfte var att utforska hur kliniskt verksamma behandlare beskriver suicidriskbedömningens praktik: genomförandet av suicidriskbedömningar, vilka processer och överväganden som vägleder bedömningen och vilka faktorer som påverkar bedömningen. En fokusgrupp med sex deltagare samt sex semistrukturerade individualintervjuer genomfördes, de tolv deltagarna var personer med olika yrken som alla var verksamma inom vuxenpsykiatrisk öppenvård och återkommande genomförde suicidriskbedömningar. Det transkriberade materialet analyserades med tematisk analys och kritisk realistisk ansats, och fem bärande teman identifierades. Studiens huvudsakliga resultat var att suicidriskbedömningens praktik bestod av flera olika delar där bedömningen, förutom värderande av risk- och skyddsfaktorer, byggde på att behandlaren gjorde ett tolkningsarbete - av patienten, sig själv och av relationen.
Faktorer som påverkade bedömningen var behandlarens känslor, patientegenskaper, alliansens kvalitet och organisatoriska/formella krav. Resultaten diskuterades utifrån teoretiska begrepp ur den psykodynamiska traditionen, relevanta för att beskriva behandlares känslor och reaktioner samt relationen mellan behandlare och patient. Resultaten diskuterades även utifrån dess implikationer för praktik, och en slutsats var att utbildning i suicidriskbedömning förutom faktakunskap om suicid också borde innehålla praktisk träning i samtalsmetodik, ökad självmedvetenhet och ett terapeutiskt förhållningssätt. The purpose of the study was to explore how clinical practitioners describe the practice of suicidal assessment: the implementation of suicide risk assessments, the processes and considerations that guide the assessment and the factors that influence it. A focus group of six participants and six semi-structured individual interviews were conducted, the twelve participants had different occupations, and were working in adult psychiatric outpatient care conducting suicide risk assessments. The material was analyzed with thematic analysis and critical realist approach, and five core themes were identified. The main result of the study was that the practice of suicidal assessment consisted of several parts where the assessment, in addition to evaluating risk- and protective factors, was based on the therapist's interpretative work of the patient, him/herself and of the relationship. Factors that affected the assessment were the therapist's feelings, patient characteristics, alliance quality and organizational/formal requirements. Theoretical concepts from the psychodynamic tradition were used to discuss the results. The results were also discussed based on its implications for clinical practice, and a conclusion was that education in suicidal assessment, in addition to knowledge of suicide, should also include practical training in communication skills, increased self-awareness and a therapeutic approach.
