- Uppsatsens syfte var att undersöka vilka hinder terapeuter inom en psykiatrisk öppenvårdskontext möter i arbetet med Prolonged exposure (PE) – en evidensbaserad form av traumafokuserad KBT – samt vilka organisatoriska förutsättningar de ser som viktiga för att bedriva metoden framgångsrikt. Sex terapeuter som nyligen genomgått utbildning i metoden intervjuades. Intervjuerna spelades in, transkriberades och analyserades med tematisk analys. I resultaten identifierades fem huvudteman: Komplexitet, komorbiditet och undvikande; ”Det känns verkligen”; Att tvivla, tro och lita på metoden; Tiden och behovet av space; och Samsyn och kommunikation inom organisationen. Resultaten synliggjorde utmaningar kopplat till patientrelaterade faktorer, avvägningar mellan följsamhet i förhållande till patienten och manualens strukturerade upplägg, tro och tillit till behandlingsmetodens effektivitet, samt organisatoriska hinder beträffande patientbedömningar, tidsbrist och tidsprioritering. Därutöver framkom att terapeuterna såg utrymme i schemat och känslomässigt stöd från kollegor eller handledare som viktiga förutsättningar för att orka med arbetet, mot bakgrund av att PE upplevdes vara en tidskrävande och känslomässigt påfrestande metod. Five superordinate themes were identified: Complexity, comorbidity and avoidance; ”It certainly has a strong effect on you”; On doubt, faith and trust in the intervention; Time and the need for space; and Consensus and communication within the organization. The results revealed challenges related to patient factors, a tension between compliance to the structure of the treatment manual and patient-centered care, trust and faith in the effectiveness of PE, as well as organizational barriers concerning patient assessment, lack of time and time-management. Additionally, considering PE as being a time-consuming and emotionally taxing therapeutic method, the therapists reported a flexible schedule and access to emotional support from colleagues or a supervisor as important organizational prerequisites for administering PE.
