Svårigheter med emotionsreglering och alexitymi medierar relationen mellan svåra upplevelser under barndomen och kroppsliga symtom

Abstract
Somatic symptoms are commonly associated with psychological factors, and their management presents considerable challenge to healthcare systems. Past research has shown simple associations between adverse childhood experiences (ACEs), alexithymia, emotion regulation difficulties, and somatic symptoms. Somatic symptoms are commonly associated with psychological factors, and their management presents considerable challenge to healthcare systems. Past research has shown simple associations between adverse childhood experiences (ACEs), alexithymia, emotion regulation difficulties, and somatic symptoms. The purpose of this study was to investigate the aforementioned variables simultaneously, while also examining whether alexithymia and emotion regulation difficulties mediate the relationship between ACEs and somatic symptoms. 330 participants recruited through convenience sampling in Sweden completed an online survey composed of 5 scales: Adverse Childhood Experiences Scale-revised, Toronto Alexithymia Scale-20, Dysfunctional Emotion Regulation Scale-16, Traits Meta Mood Scale-24, Life Events Checklist-5 and the Patient Health Questionnaire-15.The results of the current study show strong associations between ACEs, alexithymia, emotion regulation difficulties, and somatic symptoms. Moreover, alexithymia and emotion regulation difficulties were shown to be significant mediators to the relationship between ACEs and somatic symptoms. In addition, alexithymia was also found to be a significant standalone mediator in the relationship between ACEs and emotion regulation difficulties. These results suggest that interventions geared towards improving patients’ ability to identify, name, and adaptively regulate emotions; could be effective in the treatment of somatic symptoms. An additional implication is that psychotherapy aiming to reduce somatic symptoms by focusing on emotion regulation, but not the improvement of patients’ ability to identify and name emotions, risks being less effective than if these interventions were included. Som ett led i medieringsanalysen av relationen mellan svåra upplevelser under barndomen och kroppsliga symtom bekräftades även alexitymi som en signifikant mediator i relationen mellan svåra upplevelser under barndomen och svårigheter med emotionsreglering. Resultatet antyder att interventioner som syftar till att förbättra förmågan att identifiera, benämna och adaptivt reglera känslor kan vara effektiva för att behandla kroppsliga symtom, samt att psykologiska behandlingar som syftar till att reducera kroppsliga symtom och som fokuserar på emotionsreglering men inte på att utveckla förmågan att identifiera och benämna känslor riskerar att bli mindre effektiva än om dessa komponenter också skulle ha inkluderats.
