De senaste åren har diagnosticering av psykiatriska tillstånd utifrån DSM-manualens symtomkriterier varit föremål för debatt i vilken diagnosernas validitet och nytta har ifrågasatts. Lösningar på detta problem har under de senaste årens erbjudits utifrån flera olika angreppsvinklar, både vetenskapliga och dynamiska, med en gemensam förståelse kring att diagnostisering av psykiatriska tillstånd inte är någon lätt uppgift. Diagnosen spelar en stor roll inom vården i Region Skåne. Regionen strävar efter att å ena sidan bedriva holistisk, person-centrerad vård och å andra sidan basera vården på standardiserade vårdprocesser, vilket skapar en spänning mellan de två fälten. De senaste åren har diagnosticering av psykiatriska tillstånd utifrån DSM-manualens symtomkriterier varit föremål för debatt i vilken diagnosernas validitet och nytta har ifrågasatts. Lösningar på detta problem har under de senaste årens erbjudits utifrån flera olika angreppsvinklar, både vetenskapliga och dynamiska, med en gemensam förståelse kring att diagnostisering av psykiatriska tillstånd inte är någon lätt uppgift. Diagnosen spelar en stor roll inom vården i Region Skåne. Regionen strävar efter att å ena sidan bedriva holistisk, person-centrerad vård och å andra sidan basera vården på standardiserade vårdprocesser, vilket skapar en spänning mellan de två fälten. Syftet med denna studie var att undersöka klinikers erfarenhet av denna spänning och av de dilemman som kan uppstå i arbetet med att diagnostisera psykiatriska tillstånd. Studien har omfattat fjorton djupintervjuer med psykologer och läkare och har sin utgångspunkt i kvalitativ metod utifrån grundad teori.
Analysen av materialet har genererat fyra huvudteman: Diagnosens (huvud)roll i organisationen, Jakten på det mätbara, Den försvunna livsberättelsen och Diagnosen - nytta eller skada?
Resultatet visade att rätt diagnos kan vara svårt att fastslå utifrån en bristande etiologi kring psykiatriska tillstånd och på grund av hur DSM-manualen är konstruerad utifrån symtom. Resultatet visade vidare att medan diagnosen spelar en viktig roll i den psykiatriska vården upplever klinikerna att ett starkt fokus på diagnos, både inom vården och i samhället i stort, kan vara svårt att förena med ett holistiskt synsätt och person-centrerad vård.
Diskussionen adresserar de dilemman som presenteras av klinikerna samt spänningsfältet mellan diagnosfokus och holistisk vård. (Less)

During the past years, the art of diagnosing psychiatric conditions through the DSM manual have been debated and criticized and the validity of the diagnoses have been questioned. Solutions to this problem have during the past few years been offered from several different angles, both scientific and dynamic, with the common understanding that diagnosing psychiatric conditions is no easy task. The diagnosis plays a big role in public health care in Region Skåne. The region strives on one hand to practice holistic, person-centered care, and on the other hand to base the care on standardised processes, which creates a tension between the two.

During the past years, the art of diagnosing psychiatric conditions through the DSM manual have been debated and criticized and the validity of the diagnoses have been questioned. Solutions to this problem have during the past few years been offered from several different angles, both scientific and dynamic, with the common understanding that diagnosing psychiatric conditions is no easy task. The diagnosis plays a big role in public health care in Region Skåne. The purpose of this study was to look into clinicians’ experience of this tension and dilemmas that may occur in the work of diagnosing psychiatric conditions. The study consists of fourteen in-depth interviews with psychologists and medical doctors, which have been conducted through a qualitative research standpoint based on grounded theory. The analysis has generated four main-themes: The leading role of the diagnoses within the organisation, The hunt for the measurable, The lost life-story and The diagnoses - help or harm?
The result showed that finding the right diagnoses can be difficult considering the lack of information on etiology for psychiatric conditions and due to how the DSM-manual is constructed focused mainly on symtoms. The result further showed that while the diagnoses play an important part in psychiatric health care, the clinicians experience that the strong focus on diagnoses, both in health care and in society in general, can be difficult to merge with the ambition to treat the patient from a holistic point of view.
The discussion addresses the dilemmas presented by the clinicians and the tension in public health care between the focus on diagnosis and holistic care. (Less)
