Psychodynamic Psychotherapy for Refugees and Asylum Seekers: A Qualitative Outcome Study of Group-Based and Individual Treatments for PTSD

Abstract
OBJECTIVE. How is psychodynamic psychotherapy experienced by refugees with posttraumatic stress disorder? To answer this, patients’ experiences of group and individual psychotherapy were analyzed and compared. METHOD. OBJECTIVE. How is psychodynamic psychotherapy experienced by refugees with posttraumatic stress disorder? Semi-structured interviews were carried out with 10 former patients at a Swedish outpatient clinic specialized in the treatment of patients with experiences of migration, war, and torture. 5 patients had received psychoanalytic group therapy (PAGT) and 5 had received individual psychodynamic psychotherapy (PDT) with exposure-based elements. Data were analyzed according to the principles of grounded theory, while also adopting reflexive pragmatism. Furthermore, posttraumatic stress and global distress were assessed using Posttraumatic Stress Disorder Checklist (PCL-5) and Clinical Outcomes in Routine Evaluation-Outcome Measure (CORE-OM). The quantitative data were analyzed using an independent samples t-test. RESULTS. PAGT was experienced as ‘building a home’, whereas PDT was experienced as ‘being reborn’. The experiences of individual psychotherapy suggested that it was helpful in the treatment of PTSD, whereas group psychotherapy involved a sense of belongingness and closeness within the group. However, there were no significant differences between group and individual psychotherapy on PCL-5 or CORE-OM. CONCLUSION. Elaborating on previous qualitative outcome studies, psychotherapy was experienced as both helpful and painful, widening the current knowledge of evidence-based research. (Less)

Istället medförde gruppterapi en känsla av samhörighet och närhet inom gruppen. Därigenom kunde kvalitativa skillnader skönjas mellan terapimodaliteterna, där PDT upplevdes som ‘att pånyttfödas’ och PAGT upplevdes som att ‘bygga ett hem’. Det fanns dock inga signifikanta skillnader mellan grupp- och individuell psykoterapi på PCL-5 eller CORE-OM. SLUTSATS. Uppsatsen vidgar den nuvarande kunskapen om evidensbaserad forskning genom att bidra med kvalitativ kunskap om hur förändring upplevs både som hjälpsamt och som smärtsamt för patienten. (Less)
