“Jag behövde inte berätta hela min historia, hon bara visste” - Kvinnors förlossningsupplevelser i kontinuitetsvård

Bakgrund: Under 2020 ägde cirka 112 902 förlossningar rum i Sverige. Vården i samband med graviditet och förlossning är fragmenterad och inte organiserad för att skapa kontinuitet. Barnmorskeledd kontinuitetsvård innebär att en grupp barnmorskor vårdar kvinnor genom graviditet, förlossning och eftervård. Kontinuitetsmodellen caseload är internationellt vedertagen men finns i Sverige endast tillgänglig i Stockholm genom Min Barnmorska. Tidigare studier beskriver goda effekter av kontinuitetsvård under barnafödande.

Metod: Kvalitativ intervjustudie där tolv kvinnor deltog. Innehållsanalys enligt Graneheim och Lundman (2004).
Resultat: Innehållsanalysen resulterade i ett tema; Viktigast av allt är tryggheten med tre underliggande kategorier; Närvarons betydelse, Mångfacetterad kommunikation och En ömsesidig tillit.
Diskussion och konklusion: Relationen, som byggt upp under graviditeten, mellan kvinna och barnmorska har betydelse för kvinnans förlossningsupplevelse. Ett gott stöd i latensfasen och oavbruten närvaro på förlossningsrummet av en och samma barnmorska bidrar, oavsett tidigare relation, till en positiv förlossningsupplevelse. Behov ses av ytterligare forskning kring vilka faktorer som bidrar till trygghet för den födande kvinnan. Även om kontinuitet som överbryggar graviditet och förlossning kan vara svår att uppnå på en nationell nivå, ses utvecklingsmöjligheter inom förlossningsvården. (Less)

The meaning of midwife led continuity of care is that a group of midwives care for a group of women through pregnancy, delivery and postpartum care. The continuity model of care, caseload, is internationally established but in Sweden the model is only offered in Stockholm within the project My Midwife (Min Barnmorska). Studies show benefits from continuity of care in the context of childbirth.

Result: The content analysis resulted in one overarching theme; Security was of highest importance with three underlying categories; The meaning of presence, Multifaceted communication and A mutual trust.
Discussion and conclusion: The relationship, built during pregnancy, between woman and midwife has meaning for the woman’s childbirth experience. A good support during the latent phase and an uninterrupted presence of one and the same midwife in the delivery room during the active phase of labor, regardless of previous relationship, provides a good birth experience. There is a need for further research on what factors contribute to a sense of security during childbirth. Although continuity of care, during pregnancy and childbirth, can be difficult to achieve at national level, opportunities are recognized for development within childbirth care in Sweden. (Less)
