“Svårare för mig”: En tematisk analys om självomhändertagande hos personer med självskadebeteende

Abstract
Recently, preliminary research has suggested that people with deliberate self- harm (DSH) and other psychiatric disorders have a lower functioning regarding fundamental self-care activities, compared to people with psychiatric disorders but without DSH. However, the reasons for this have not been studied in previous research. An explorative qualitative research study with a critical realist approach was conducted with the aim of investigating how people with DSH, who receive specialized psychiatric care, experience their self-care functioning and what conditions are perceived to facilitate, complicate and improve their self-care functioning. Recently, preliminary research has suggested that people with deliberate self- harm (DSH) and other psychiatric disorders have a lower functioning regarding fundamental self-care activities, compared to people with psychiatric disorders but without DSH. However, the reasons for this have not been studied in previous research. An explorative qualitative research study with a critical realist approach was conducted with the aim of investigating how people with DSH, who receive specialized psychiatric care, experience their self-care functioning and what conditions are perceived to facilitate, complicate and improve their self-care functioning. Participants were recruited from a psychiatric unit specialized in treating self-harm. Twelve participants, all of whom had self-harmed within the last six months, participated in semi- structured interviews on the topic of self-care. The transcribed material was processed using reflexive thematic analysis and four main themes were generated: (1) Self-care is a difficult choice for me, (2) Self-care is beyond my control, (3) Why should I choose self-care? and (4) Supported self-care both helps and hinders me. The study shows that self-care can be experienced as a difficult, unclear and dichotomous choice, rather than as an obvious part of everyday life. A multifaceted need for care and support that went beyond DSH was identified and several possible reasons for the perceived functional disability were reported. Nyligen har preliminär forskning funnit att personer med självskadebeteende och andra psykiatriska diagnoser verkar ha en lägre funktionsnivå avseende grundläggande självomhändertagande aktiviteter, jämfört med personer med psykiatriska diagnoser men utan självskadebeteende. Anledningarna till detta har dock inte studerats i tidigare forskning. En explorativ kvalitativ forskningsstudie med en kritisk realistisk ansats genomfördes i syfte att undersöka hur personer med självskadebeteende som får specialistpsykiatrisk vård upplever sin funktionsnivå avseende självomhändertagande samt vilka omständigheter som upplevs underlätta och försvåra självomhändertagande, respektive öka funktionsnivån avseende självomhändertagande. Deltagare rekryterades från en psykiatrisk enhet som är specialiserad inom behandling av självskadebeteende. Tolv deltagare, som alla hade självskadat det senaste halvåret, deltog i semistrukturerade intervjuer om självomhändertagande. Det transkriberade materialet analyserades genom reflexiv tematisk analys och fyra huvudteman genererades: (1) Självomhändertagande är ett svårt val för mig, (2) Självomhändertagande är bortom min kontroll, (3) Varför ska jag välja självomhändertagande? och (4) Medomhändertagande både hjälper och stjälper mig. Studien visar att självomhändertagande kan upplevas som ett svårt, otydligt och dikotomt val för deltagarna, snarare än som en självklar del av vardagen. Ett mångfacetterat vård- och stödbehov som gick bortom självskadebeteende framkom och flera möjliga anledningar till den upplevda funktionsnedsättningen rapporterades.
