"Det hade kanske kunnat bli bra": En kvalitativ analys av behandlingsmisslyckanden i KBT respektive PDT för paniksyndrom

Studiens syfte var att utforska upplevda metodbrister hos patienter som inte blivit hjälpta av Panic Control Treatment (PCT) respektive Panic Focused Psychodynamic Psychotherapy (PFPP). Studiens urval var en del av projektet POSE, Psychotherapy outcome and self-selection effects for panic disorder, och bestod av 20 patienter med diagnosen panikångestsyndrom med eller utan agorafobi. Hälften hade behandlats med PCT och hälften med PFPP. Ingen hade nått en kliniskt meningsfull förbättring. Patienterna intervjuades vid uppföljning sex månader efter behandlingens avslut. Resultatet visade på både gemensamma och särskiljande upplevelser. Gemensamma upplevda metodbrister var att terapin var otillräcklig för att kunna nå önskade resultat och att metoden på olika sätt ställde för höga krav i relation till den egna förmågan. Hos PFPP-patienter fanns upplevelsen av övergivenhet, genom att metoden var svårbegriplig och avslutet alltför smärtsamt. PCT-patienterna upplevde att behandlingen hade alltför snäva ramar, varav det saknats utrymme för att på djupet förstå sina svårigheter och dess orsaker. Studien understryker vikten av att terapeuter är noga med att utforma överenskommelser om behandlingens mål och arbetssätt. (Less)

experiences included that the therapy was not enough to reach desired goals and
that the demands of therapy did not match the patient’s estimation of their own
ability. PFPP patients experienced abandonment in terms of the interventions
being too abstract and the termination of therapy too painful. PCT patients
experienced that the treatment had too rigid frameworks and that there was no
room for in-depth understanding of their problems. The study underscores the
importance of carefully forming an agreement of goal and task. (Less)
