Abstract
The aim of this study was to investigate how hospital counselors in specialized palliative care reason about their discretion and their professional role in the multi-professional team, in order to deepen the understanding of their professional role as well as the psychosocial work that is carried out within the medical context. The aim of this study was to investigate how hospital counselors in specialized palliative care reason about their discretion and their professional role in the multi-professional team, in order to deepen the understanding of their professional role as well as the psychosocial work that is carried out within the medical context. The methodology was based on a qualitative approach using seven semi-structured interviews with healthcare counselors working in specialized palliative care in the south of Sweden. The results were analyzed using thematic analysis. The results were understood through previous research on the subject and the following theoretical framework: Goffman and the dramaturgical theory, Lipsky’s theory of discretion and street-level bureaucracy as well as Abbott’s theory of jurisdiction. The results show that the counselors mostly consider themselves highly trusted by both managers and other staff in the multi-professional medical team. They feel like they have substantial discretion and the opportunity to individually shape their services and tasks. They all see a greater need for psychosocial work in their units than they can accomplish with the resources available today. The discretion therefore can be seen as limited by the organizational frame. Despite the high level of trust, several of the counselors believe that other staff do not always know what their professional knowledge is and what they do. This essay has shown, among other things, that the interviewed healthcare counselors create validity and credibility by being visible in the daily work of the multi-professional team, which also increases the clarity of the role of social workers in a medical context. This is a very small study of some counselors reasoning on the subject and therefore cannot claim to have any comprehensive answers. More research is needed to investigate the role of the healthcare counselor in the hospital and in specialized palliative care to further deepen the understanding of how psychosocial work is carried out within the care units and how multi-professional collaboration can increase.
