Abstract
Intergenerational trauma, how previous generations traumatic experiences effect the coming generations, has become an increasing area of interest for different disciplines and is relevant for psychotherapists. The aim of this study was to explore psychodynamic oriented psychotherapists understanding and work with intergenerational trauma within a qualitative framework. Five psychotherapists experiences were examined using semi-structured interviews and analyzed with Interpretative phenomenological analysis (IPA). Intergenerational trauma, how previous generations traumatic experiences effect the coming generations, has become an increasing area of interest for different disciplines and is relevant for psychotherapists. The aim of this study was to explore psychodynamic oriented psychotherapists understanding and work with intergenerational trauma within a qualitative framework. Five psychotherapists experiences were examined using semi-structured interviews and analyzed with Interpretative phenomenological analysis (IPA). Four main were identified: Dilemmas in conceptualization, Traumatized parenting, Expressions in the therapy room and Beyond the individual. The results were interpreted and discussed in relation to psychoanalytic theory and previous research. The results showed that effects of intergenerational trauma were hard to define, conceptualize and differ from self-experienced traumatic experiences. The role of meticulous assessment of the individual patient was highlighted. It was indicated that modes of transmission of trauma took place in the relationship between the parent and child on an unconscious bodily level. Different forms of harmful family communications like silences were brought up together with potential parental neglect, dissociation and deficient affect regulation. The results presented how effects of intergenerational trauma could be seen in clinical symptoms. Narrative interventions with the aim to increase the capacity for symbolization was highlighted as important. Thoughts about how society could help the processing of trauma was pointed out. One conclusion is that effects of intergenerational trauma is very subjective, and the concept can be used as a helpful hypothesis in clinical thinking and should not be treated as a fixed diagnostic category. Resultaten visade att fenomenet intergenerationella trauman var komplext och svårt att särskilja från egenupplevd traumatisering. Vikten av subjektivitet och noggrann bedömning av varje enskild patient lyftes fram. Resultaten visade även att psykoterapeuterna förstod överföring av trauma som en omedveten kommunikation vilken skedde i det relationella samspelet mellan förälder och barn. Olika former av skadlig familjekommunikation såsom tystnad eller invadering lyftes också fram i resultaten tillsammans med traumatiserade föräldrars potentiella omsorgssvikt, dissociation och bristande affektreglering. Resultaten visade hur intergenerationell traumatisering kunde förstås i relation till kliniska symtom med missbruk och somatisering som exempel. Narrativa interventioner med syfte att öka mentaliserings- och symboliseringsförmågan framhölls som viktiga terapeutiska mekanismer. Tankar om hur det omgivande samhället kan vara en plats för bearbetning av trauma lyftes också upp. Diskussion förs kring positionerna offer och förövare i relation till föräldraskap samt om trauma som begrepp. En slutsats är att effekterna av intergenerationell traumatisering är subjektiva och att konceptet kan användas som en hjälpsam hypotes i kliniskt tänkande.
