Fungerar familjeterapi som behandling för depression hos barn och ungdomar? En systematisk litteraturstudie

Abstract
The aim of this literature study was to examine the evidence of family therapy and familybased interventions for children and adolescents with depression. Additionally, this study highlights how different research perspectives and outcome measures may affect the recommendations of treatment interventions. Nine articles were reviewed and assessed for their scientific quality. Five of the articles were randomized controlled studies (RCTs) and four studies were of a different design (non-RCTs). The aim of this literature study was to examine the evidence of family therapy and familybased interventions for children and adolescents with depression. Additionally, this study highlights how different research perspectives and outcome measures may affect the recommendations of treatment interventions. Nine articles were reviewed and assessed for their scientific quality. Five of the articles were randomized controlled studies (RCTs) and four studies were of a different design (non-RCTs). The overall results of the studies showed that family therapy or family-based interventions reduce depression symptoms. When compared to other treatments (not all studies included comparison with other treatments) the intervention reduced the depression symptoms equivalently to other treatments and in some cases more effectively. Positive side effects on comorbidity as well as familial and environmental factors were also shown in some of the studies. Given the variety and the modest overall quality of the studies some of the results should be interpreted with great caution. Similar results are supported in some meta-analyses but questioned in others. The conclusion of this literature study was that there is evidence to support the recommendation and choice of family therapy in the treatment of depression in children and adolescents on the
basis of the Evidence Based Practise (EBP) perspective when taking efficacy, effectiveness and efficiency in consideration. More family therapy studies of higher quality and on the basis of EBP are needed measuring other domains than symptoms of the primary diagnosis. Manualizing family therapy methods or conceptualizing common factors may be an important step forward to identify the specific mechanisms of change in evidence and to be able to generalize them. Syftet med denna litteraturstudie var att undersöka det vetenskapliga stödet for familjeterapi och familjebaserade interventioner vid behandling för depression hos barn och ungdomar. Dessutom ville vi belysa hur olika forskningsperspektiv och utfallsmått kan påverka rekommendationerna för val av behandling. Nio artiklar undersöktes och bedömdes efter dess vetenskapliga kvalitet. Fem av artiklarna var randomiserade kontrollerade studier (RCT) och fyra var studier av annan design. Syftet med denna litteraturstudie var att undersöka det vetenskapliga stödet for familjeterapi och familjebaserade interventioner vid behandling för depression hos barn och ungdomar. Dessutom ville vi belysa hur olika forskningsperspektiv och utfallsmått kan påverka rekommendationerna för val av behandling. Nio artiklar undersöktes och bedömdes efter dess vetenskapliga kvalitet. Fem av artiklarna var randomiserade kontrollerade studier (RCT) och fyra var studier av annan design. Sammanfattningsvis visade resultaten i studierna att familjeterapi eller familjebaserade interventioner ledde till minskning av depressionssymtom, oftast med likvärdig effekt som annan behandling, i de fall där det gavs, och ibland med bättre effekt. Resultat i form av positiva bieffekter på komorbida symptom samt på familj och andra omgivande faktorer kunde också visas i några av studierna. Resultaten behöver dock tolkas med stor försiktighet med tanke på några av studiernas skiftande och måttliga kvalitet. Liknande resultat stöds av vissa metaanalyser men ifrågasätts i andra. Slutsatsen av denna litteraturstudie var att det finns evidens för att kunna rekommendera valet av familjeterapi som behandling vid depression av barn och ungdomar när EBP (Evidensbaserad praktik) får utgöra grundvalen och när de tre effektmåtten efficacy, effectiveness och efficiency tas i beaktande. Det behövs fler högkvalitativa studier på familjeterapi med avstamp i EBP och med fler utfallsmått än symtomeffekt på den primära diagnosen. Att manualisera familjeterapimetoder eller konceptualisera s.k. common factors skulle kunna vara ett viktigt steg mot att kunna identifiera och generalisera specifika förändringsmekanismer. (Less)
