Den här studien var en del av forskningsprojektet “Bemötandebrister i vuxenpsykiatrisk vård” vid Lunds universitet. Frågeställningen var “Vilken bild har representanter för patient- och anhörigorganisationer av bemötandebrister i svensk vuxenpsykiatri, baserat på medlemmarnas erfarenheter?”. Två fokusgrupper, med sju representanter för olika svenska patient- och anhörigorganisationer, genomfördes. Materialet analyserades sedan genom en kritisk realistisk reflexiv tematisk analys, vilket genererade fyra huvudsakliga teman: Psykiatrin är otillgänglig, Det blir inget samarbete, Saknas helhetsperspektiv på människan samt En sårbar individ möter en stark psykiatri. Den här studien var en del av forskningsprojektet “Bemötandebrister i vuxenpsykiatrisk vård” vid Lunds universitet. Frågeställningen var “Vilken bild har representanter för patient- och anhörigorganisationer av bemötandebrister i svensk vuxenpsykiatri, baserat på medlemmarnas erfarenheter?”. Två fokusgrupper, med sju representanter för olika svenska patient- och anhörigorganisationer, genomfördes. Materialet analyserades sedan genom en kritisk realistisk reflexiv tematisk analys, vilket genererade fyra huvudsakliga teman: Psykiatrin är otillgänglig, Det blir inget samarbete, Saknas helhetsperspektiv på människan samt En sårbar individ möter en stark psykiatri. Deltagarna beskrev hur svårigheter med tillgången till psykiatrisk vård kunde uppfattas som en bemötandebrist i sig. De beskrev också att det bland patienter finns upplevelser av att inte ses som en resurs, samtidigt som de och anhöriga lämnas ensamma med det huvudsakliga ansvaret för vården. Många uppfattade också ett alltför stort fokus på enskilda diagnoser eller på förhand bestämda metoder och vårdprocesser framför patientens livshistoria. Slutligen beskrevs hur patienter upplever sig avfärdade och ifrågasatta i psykiatrin, vilket resulterar i negativa konsekvenser. I relation till teorier om exempelvis makt, autonomi och deumanisering tycks bilden av bemötandebrister vara att det snarare rör sig om en hållning och människosyn än en lista på konkreta beteenden, att bristerna både finns inbyggda i systemet och sker i de individuella mötena samt att ett gott bemötande kan förstås som en verksam intervention i sig och att motsatsen därför kan ha en negativ inverkan på behandlingsutfall. Studien belyser aspekter som kan vara hjälpsamma för kliniker och beslutsfattare att ha i åtanke. (Less)

This study was part of the research project “Violations* in adult psychiatric care” at Lund University. The research question was “What is the view, held by patient organizations, of violations* within Swedish adult psychiatry, based on their members’ experiences?”. Two focus groups were conducted with a total number of seven representatives from different Swedish patients’ organizations. The data was analyzed using critical realist reflexive thematic analysis, generating four main themes: The psychiatry is inaccessible, There is no collaboration, Lacking a holistic perspective on the human being and A vulnerable individual meets a powerful psychiatry. This study was part of the research project “Violations* in adult psychiatric care” at Lund University. The research question was “What is the view, held by patient organizations, of violations* within Swedish adult psychiatry, based on their members’ experiences?”. Two focus groups were conducted with a total number of seven representatives from different Swedish patients’ organizations. The data was analyzed using critical realist reflexive thematic analysis, generating four main themes: The psychiatry is inaccessible, There is no collaboration, Lacking a holistic perspective on the human being and A vulnerable individual meets a powerful psychiatry. The participants described how difficulties accessing psychiatric care could be considered a violation* in itself. They also described experiences among patients of not being treated as a resource, while at the same time being left with sole responsibility for the care. Many also experienced a preoccupation with specific diagnosis and predetermined methods, rather than the patient’s life history. Finally, the participants described that many patients feel questioned seeking psychiatric care, resulting in negative consequences. Discussed in relation to theories regarding such concepts as autonomy, power and dehumanization, it seems that the view of violations concerns a general attitude, rather than a list of behaviours. The violations seem to be built into the system as well as something that happens in the individual encounter and being treated with respect can be regarded as an intervention in itself. The study highlights aspects that can be helpful to clinicians and policymakers.

* Note that the word originally used in this study was the Swedish word “bemötandebrister”, which roughly translates to “abusive behaviour” or “violations”, but with a slightly broader meaning. The word “bemötandebrister” can refer to less severe cases, such as a treatment lacking in kindness, as well. (Less)
