Studiens syfte var att undersöka vilka patienter med depressionsdiagnos som
behandlats med individuell psykoterapi hos psykolog inom psykiatriska
öppenvården vid Psykiatri Affektiva, Sahlgrenska Universitetssjukhuset åren
2013 till 2023. Data inhämtades från klinikens patientadministrativa system där
vårdåtgärder registreras och innefattade 27 008 patienter. Patienter med
depressionsdiagnos gick i psykoterapi i större utsträckning än patienter med
andra diagnoser. Resultaten visade att av totalt 2909 patienter med
depressionsdiagnos hade 16.2% erhållit psykoterapi jämfört med 12.9% av
24 099 patienter med andra diagnoser. Frågan om jämlik vård belystes genom

andra diagnoser. Resultaten visade att av totalt 2909 patienter med

24 099 patienter med andra diagnoser. Frågan om jämlik vård belystes genom
att undersöka skillnader i andel som behandlats med psykoterapi utifrån kön och
ålder. När patienter med depression jämfördes avseende psykoterapeutisk
behandling framkom signifikanta skillnader avseende ålder men ej avseende
kön. Således var det större sannolikhet att yngre patienter behandlades med
psykoterapi. Vidare framkom att patienter som vårdats inneliggande behandlats
med psykoterapi i större utsträckning än de som inte gjort det. Studien visade
en diskrepans mellan nationella riktlinjerna för depressionsvård och den
kliniska vardagen avseende psykoterapeutisk behandling. För det första fick låg
andel patienter med depression psykoterapi. För det andra påverkade grad av
depression inte sannolikheten att gå i psykoterapi. Slutligen att psykodynamisk
terapi och ”annan psykologisk behandling” var mer vanligt förekommande än
KBT. Resultatet visar behovet att vidare undersöka orsakerna till skillnaderna.
Studien kan ge insikter om utmaningar för psykiatrisk specialistvård i
implementeringen av riktlinjer. Utmaningar att utifrån vårdens förutsättningar
försöka väga samman patientens unika omständigheter med bästa tillgängliga
evidens för att på lika villkor möta patientens behov av förbättrad hälsa och

outpatient care at Psykiatri Affektiva, Sahlgrenska Universitetssjukhuset,

Furthermore, it emerged that patients who received inpatient care received
psychotherapy to a greater extent than those who did not. The study showed a
discrepancy between the national guidelines for depression care and everyday
clinical practice regarding psychotherapy. First, a low proportion of patients
with depression attended psychotherapy. Second, degree of depression did not
affect the likelihood of psychotherapeutic treatment. Finally, psychodynamic
therapy and” other psychological treatment” were more common than CBT. The
result shows the need to further investigate the reasons for the differences. The
study may provide insights into challenges for psychiatric specialist care in the
implementation of guidelines. How, based on society’s conditions, to weigh the
patient’s unique circumstances with best available evidence to be able to meet
the patient’s need for improved health and quality of life on equal terms. (Less)
