Abstract
This study aims to investigate how patients, who are enrolled in a rehabilitation clinic in outpatient care, and their relatives' views on working from a rehabilitation plan. This was done by examining how patients and relatives describe it. The study was conducted through semi-structured interviews with six patients and three relatives, who are enrolled in an outpatient department and work based on a rehabilitation plan. The patients all have neurological damage, either due to injury or disease. The thesis has had a humanistic approach to interpreting and understanding the information. This study aims to investigate how patients, who are enrolled in a rehabilitation clinic in outpatient care, and their relatives' views on working from a rehabilitation plan. This was done by examining how patients and relatives describe it. The study was conducted through semi-structured interviews with six patients and three relatives, who are enrolled in an outpatient department and work based on a rehabilitation plan. The patients all have neurological damage, either due to injury or disease. The thesis has had a humanistic approach to interpreting and understanding the information. The theories used are person-centered care, Carl Rogers' theory of client-centered therapy, and Aaron Antonovsky's KASAM (Sense of Coherence). The results show that both patients and relatives are very satisfied with the care, where the study is carried out. The study shows that the rehabilitation plan has proved to be clear and easy to understand for both patients and relatives. They participated in setting the goals and were well supported by the whole team. It does not appear that the team has taken over the patient's voice in the rehabilitation plan; patients feel that it is their words. However, it has emerged that the plan itself, as a formal document, is not well used by most patients, but the majority know what they are going to achieve and what they are going to do along the way. Patients have also been helped by the plan in their daily lives, as confirmed by family members. More follow-up joint meetings have been expressed as a wish by almost everyone, which could contribute to an even more holistic and individualized care environment where the patient's needs and wishes are prioritized.
