Psychological Flexibility and Daily Functioning in Adults with Systemic Lupus Erythematosus: a Daily Diary Study

The primary aim of this study was to assess the feasibility of obtaining twice-daily recordings of symptoms, psychological flexibility (PF) and functioning from adults with Systemic Lupus Erythematosus (SLE). The secondary aim was to investigate the relationship between the most common and burdensome symptoms of SLE (i.e., pain, fatigue, anxiety, depression and cognitive impairment – CI), health related quality of life (HRQoL), and both psychological flexibility (PF) and psychological inflexibility (PiF), at both the group and individual level. Twenty-four participants with SLE were primarily recruited from online SLE support forums and completed a pre-diary assessment (baseline) involving standardized self-report measures of pain, fatigue, anxiety, depression, PiF, and HRQoL; followed by a three-week daily diary phase involving twice daily self-reports of the severity and interference from pain, fatigue, emotional distress and cognitive impairments (CIs), and PF. At the end of the study, participants completed items relating to their experience of participation. Data were analyzed using summary statistics, correlations at the individual and group level, and multilevel analysis using linear mixed models (LMM) due to the hierarchical structure of the daily diary data. The number of participants recruited and the completion rate of daily diaries was poorer than expected, suggesting that daily diary recordings may be perceived as a significant burden to adults with SLE. Participants who did complete the diaries indicated an overall positive experience of study participation. Consistent with expectations, lower PiF at baseline was associated with lower levels of anxiety, depression, and fatigue, and higher levels of HRQoL at the group level. In analyses of diary data using LMM, and consistent with expectations, higher levels of PF appeared to moderate the relationship between the severity and interference from fatigue, emotional distress and CI’s, at the group and individual level. No such relationship was found for pain, perhaps owing to floor effects. (Less)

Det primära syftet med studien var att undersöka genomförbarheten av dagliga skattningar två gånger dagligen av symtom, psykologisk flexibilitet (PF) och funktion hos vuxna med systemisk lupus erythematosus (SLE). Data analyserades med hjälp av sammanfattande statistik, korrelationer på individ- och gruppnivå samt multinivå-analyser med linear mixed model (LMM) på grund av den hierarkiska strukturen i de dagliga skattningarna. Antalet deltagare som rekryterades och svarsfrekvensen i de dagliga skattningarna var lägre än förväntat, vilket tyder på att de dagliga skattningarna kan ha upplevts som en betydande börda för vuxna med SLE. De som fullföljde perioden med dagliga skattningar indikerade en överlag positiv upplevelse av studiedeltagandet. I enlighet med förväntningarna var lägre nivåer av PiF vid baslinjemätningen relaterad till lägre nivåer av ångest, depression och trötthet, samt högre nivåer av hälsorelaterad livskvalité på gruppnivå. I analyserna av de dagliga skattningarna med LMM, och i enlighet med förväntningarna, tycktes högre nivåer av PF moderera sambandet mellan svårighetsgrad och påverkan av trötthet, känslomässigt lidande och CIs, på både grupp och individnivå. Ett sådant samband hittades inte för smärta, eventuellt på grund av golveffekter. (Less)
