Abstract
When every individual’s right to self-determination is viewed against ethical considerations and wishes to protect people from harm, professionals face challenges in their work with patients living with dementia. This study aimed to illustrate health social workers’ approach to legislation regulating self-determination and how this affects their work with patients living with dementia. To investigate this, a qualitative method was used, consisting of eight semi-structured interviews with health social workers who work with patients living with dementia and their relatives. When every individual’s right to self-determination is viewed against ethical considerations and wishes to protect people from harm, professionals face challenges in their work with patients living with dementia. This study aimed to illustrate health social workers’ approach to legislation regulating self-determination and how this affects their work with patients living with dementia. To investigate this, a qualitative method was used, consisting of eight semi-structured interviews with health social workers who work with patients living with dementia and their relatives. The results were analyzed using a thematic approach. The findings reveal that health social workers encounter dilemmas in several aspects related to self-determination. The central concepts that affect health social workers’ attitudes include different perspectives on how cognitive impairment affects patients’ ability to exercise self-determination. The health social workers identified challenges in maintaining the right to self-determination. Mainly, the challenges consist of self-neglect, the strain on the patients’ relatives, and the patients’ networks. The study concludes that the legislation on self-determination is not complied with. This is a complex problem that puts the health social worker in a difficult position, the problem is hard to solve. (Less)
