Vilka förbättringsområden identifierar vårdpersonal i svensk transvård i den egna verksamheten?

A large-scale international report recently published by Transgender Europe shows great discrepancy between the patients’ and the caregivers’ views on transgender care. Apart from this report, those working in transgender care have yet to make their voices heard. However, transgender individuals as well as organizations working with issues of the transgender and queer communities in Sweden have been voicing concerns regarding both medical staff competence and the current design of transgender care. A large-scale international report recently published by Transgender Europe shows great discrepancy between the patients’ and the caregivers’ views on transgender care. Apart from this report, those working in transgender care have yet to make their voices heard. However, transgender individuals as well as organizations working with issues of the transgender and queer communities in Sweden have been voicing concerns regarding both medical staff competence and the current design of transgender care. Hence, the purpose of this study is to call attention to the caregivers’ perspective. What areas of improvement do people working in Swedish transgender care identify in their own field? How content are they with the health care provided?

A survey was designed, asking the participants to estimate their satisfaction with the health care provided at their respective units, and what areas of improvement they could identify in their own field. The survey was sent to all Swedish health care units providing transgender care. Thirty-nine professionals, working at seven of the eight units, participated. These included doctors, nurses, psychologists, therapists, assistant nurses, speech therapists and administrative staff.

Among the participants, the most commonly identified area of improvement was increasing the financial resources. Several participants were also frustrated with the scarcity of research on the subject. In spite of these seemingly major issues, eighty percent of the participants considered themselves content with the health care provided at their respective units.

In conclusion, increased resources and intensified research on different aspects of transgender care are the most important areas of improvement, as perceived by Swedish professionals. Nevertheless, the majority of those working in Swedish transgender care are largely content with the care currently provided. (Less)

De senaste åren har svensk transvård varit ämne för skarp kritik från såväl enskilda transpersoner som organisationer engagerade i transfrågor. Hösten 2017 publicerade Transgender Europe en omfattande internationell rapport som visar på betydande skillnader mellan patienters och vårdgivares uppfattning av den vård som erbjuds transpersoner i bland annat Sverige. Förutom i denna rapport har det dock sällan framgått vad de anställda i svensk transvård anser om den verksamhet som finns på landets mottagningar. Mot denna bakgrund syftar denna studie till att uppmärksamma vårdgivarnas perspektiv på den egna verksamheten. Vilka förbättringsområden identifierar vårdpersonalen i sin verksamhet? En enkät utformades i syfte att låta vårdgivarna delge sitt perspektiv. Deltagande erbjöds samtliga anställda i svensk transvård, och sju av åtta mottagningar valde att delta. Bland de totalt 39 deltagarna fanns läkare, psykologer, sjuksköterskor, undersköterskor, logopeder, kuratorer och medicinska sekreterare.

Resultaten visar att det förbättringsområde flest anställda i transvården identifierar är tillgången till resurser. Detta, i kombination med en påtaglig brist på forskning och beprövad erfarenhet, uppges vara de största orsakerna till missnöje bland såväl patienter som vårdgivare. En stor del av vårdgivarna upplever dock att både de själva och en majoritet av patienterna är nöjda med nuvarande verksamhet. För att utveckla den svenska transvården föreslås bland annat kontinuerlig fortbildning för samtliga anställda, uppdatering av nuvarande metoder för könsidentitetsutredningar, och långsiktig uppföljning av patienternas psykiska hälsa.

Slutsatsen är därmed att personal i transvården i hög utsträckning är nöjda med den vård som erbjuds transpersoner i Sverige. De förbättringsområden de identifierar i sin verksamhet är ökade resurser och mer forskning, kontinuerlig fortbildning samt viss revidering av könsidentitetsutredningarnas utformning. Att uppmärksamma dessa av personalen identifierade utvecklingsområden är ett viktigt steg i utvecklandet av en bättre svensk transvård. (Less)
