Bakgrund: Evidensbaserad vård har visat sig ha positiva effekter för både patienter och organisationen, och är ett väl genomforskat ämne i bland annat sjukhuskontext. Det finns barriärer relaterat till dess implementation som utgörs av exempelvis brist på tid, resurser och kunskap. Evidensbaserad vård inom ambulanssjukvård är dock inte lika väl undersökt, vilket understryker ett behov av en utforskande studie. Det finns barriärer relaterat till dess implementation som utgörs av exempelvis brist på tid, resurser och kunskap. Evidensbaserad vård inom ambulanssjukvård är dock inte lika väl undersökt, vilket understryker ett behov av en utforskande studie. Syfte: Att utforska specialistsjuksköterskors upplevelser av evidensbaserad vård svensk ambulanssjukvård. Metod: Studien utgick från en kvalitativ ansats och data insamlades genom semistrukturerade intervjuer. Urvalet bestod av specialistsjuksköterskor i ambulanssjukvården. Insamlad data transkriberades ordagrant och analyserades induktivt genom kvalitativ innehållsanalys. Resultat: Nio kategorier framkom: Evidens är mångfacetterad; Syftet med evidensbaserad vård; Källor till evidens; Chefsrollen, handlingar och koppling till förändring; Arbetsplatsens tillgänglighet för evidens; Nytta och onytta av utbildning; Betydelsen av individuella skillnader; Arbetskulturens påverkan samt slutligen Behovet för, samt betydelsen av, underlättande roller. Attityder gentemot evidensbaserad vård var både positiva och negativa, chefs- och underlättarrollen hade en viktig funktion samt individuella skillnader spelade stor roll. Slutsats: Liknande hinder som fanns i tidigare forskning finns även inom den svenska ambulanssjukvården och liknande insatser bör även fungera i detta sammanhang. Om man hade enbart behandlingsriktlinjer som källa till evidens kan det vara en barriär för god omvårdnad, då det ej innefattas av behandlingsriktlinjerna. Ytterligare kvantitativ forskning behövs för att mer fullständigt förklara evidensbaserad vård inom den svenska ambulanssjukvården samt riktade interventioner för att överkomma de barriärer som identifierats. There are however barriers pertaining to its implementation in the form of, for example, lack of time, resources, and knowledge. Evidence-based practice in the ambulance service context is, however, not nearly as well researched, which highlights a need for an exploratory study. There are however barriers pertaining to its implementation in the form of, for example, lack of time, resources, and knowledge. Evidence-based practice in the ambulance service context is, however, not nearly as well researched, which highlights a need for an exploratory study. Aim: To explore specialist nurses’ experiences of Evidence-based Practice in Swedish ambulance services. Methods: A qualitative approach was chosen, and specialist nurses working in the ambulance services were sampled. Semi-structured interviews and an interview guide were utilized, and the resulting data was transcribed and later analyzed using inductive content analysis. Findings: Nine categories were found: Evidence is multifaceted; Purpose of Evidence-based Practice; Sources of evidence; Managerial role, actions, and connection to change; Workplace availability of evidence; Benefits and non-benefits of education; Significance of individual differences; Influence of work culture; and The need for, and importance of, facilitators. Attitudes towards Evidence-based Practice were both good and bad, managerial, and facilitating roles carried an important function, individual characteristics carried increased importance. Conclusion: Similar barriers as was found in previous research exist also in Swedish ambulance services, and similar interventions should also function in this context. Additionally, if one used treatment guidelines as a major source of evidence one would risk not utilizing nursing research as part of one’s evidence base, thus lowering the quality of their nursing ability. Further quantitative research is needed to explain Evidence-Based Practice in the Swedish ambulance services more fully as well as targeted interventions to overcome the identified barriers. (Less)
