När patienter gör sig illa: en tematisk analys av psykologers syn på bemötande av självskadebeteende

Tidigare forskning har visat att personer som skadar sig själva tenderar att uppleva mötet med vården negativt. Vårdpersonal kan ha negativa attityder till självskadebeteende, och deras möten med patienter kan väcka känslor av frustration, uppgivenhet och oro, vilket kan påverka bemötandet. Studier har framförallt gjorts med sjuksköterskor och läkare, och i betydligt mindre omfattning med psykologer. I denna studie undersöktes hur psykologer förstår och upplever mötet med patienter som skadar sig själva och förutsättningarna för ett gott bemötande. Tidigare forskning har visat att personer som skadar sig själva tenderar att uppleva mötet med vården negativt. Vårdpersonal kan ha negativa attityder till självskadebeteende, och deras möten med patienter kan väcka känslor av frustration, uppgivenhet och oro, vilket kan påverka bemötandet. Studier har framförallt gjorts med sjuksköterskor och läkare, och i betydligt mindre omfattning med psykologer. I denna studie undersöktes hur psykologer förstår och upplever mötet med patienter som skadar sig själva och förutsättningarna för ett gott bemötande. I studien intervjuades elva psykologer i Sverige med erfarenhet av att möta patienter som skadar sig själva. Den tematiska analysen genererade tre teman. I det första temat, Svåra känslor och flytande gränser, speglades intervjupersonernas stora fokus på beteendets emotionsreglerande funktion, och svårigheterna att göra en entydig definition. Det andra temat, Validera, försök förstå och behåll lugnet, återgav en samsyn hos intervjupersonerna om vad som är gott bemötande och vikten av att alla i vården bemöter självskadebeteende på samma sätt. Det sista temat, Roligt och givande - med rätt förutsättningar, beskrev bland annat vikten av kollegialt stöd, god utbildning och resurser i organisationen. Avslutningsvis diskuteras resultatens implikationer för bland annat självskadebeteende som framtida klinisk diagnos och tänkbara utbildningsinsatser för vårdpersonal. Ett behov av fortsatt forskning konstaterades angående exempelvis värdet av de utbildningsinsatser som redan görs, och om psykologens och patientens samtidiga

Health service staff may have negative attitudes towards Non-Suicidal Self-injury (NSSI) and their encounters with patients may evoke frustration, hopelessness and worry, which may influence their behavior towards patients. Previous research has shown how individuals who injure themselves tend to have negative experiences from their encounters with health services. Health service staff may have negative attitudes towards Non-Suicidal Self-injury (NSSI) and their encounters with patients may evoke frustration, hopelessness and worry, which may influence their behavior towards patients. Studies have mainly focused on nurses and doctors, and much
less on psychologists. The aim of the current study was to investigate how psychologists in Sweden understand and experience their encounters with patients who injure themselves and the prerequisites for a good approach (including constructive attitudes,feelings and behavior). Eleven psychologists in Sweden, with experience of seeing patients who injure themselves, were interviewed. A thematic analysis generated three themes. The first theme, Difficult emotions and fluid boundaries, highlighted the
participants’ focus on the emotion-regulation function of NSSI, and the difficulty of defining NSSI. The second theme, Validate, try to understand and keep calm, summarized a common view among the participants about a constructive approach, and the importance for all health service staff to handle NSSI in the same way. The final theme, Fun and rewarding - given the right prerequisites, described the importance of supportive colleagues, adequate education and the right organizational resources. In the final section we discuss the implications of our results for an inclusion of NSSI syndrome in upcoming diagnostic manuals, and future staff training programs. We conclude that there is a need for further research on, for example, the efficiency of staff training, and studies where the
experiences of both psychologists and their patients are studied simultaneously. (Less)
