Abstract
For decades international research has shown that children in out-of-home care have poor dental health compared to their peers. Although new and more rigorous health regulations have been introduced to the Swedish child welfare authority system, this has not been enough to solve the problem. The aim of this study was to examine how child welfare secretaries relate to regulations concerning their responsibility for dental health needs of out-of-home placed children, as well as to study how dental needs are recognized and dealt with. Semi-structured interviews were carried out with six welfare secretaries at three child welfare units in Skåne county, Sweden. Although new and more rigorous health regulations have been introduced to the Swedish child welfare authority system, this has not been enough to solve the problem. The aim of this study was to examine how child welfare secretaries relate to regulations concerning their responsibility for dental health needs of out-of-home placed children, as well as to study how dental needs are recognized and dealt with. Semi-structured interviews were carried out with six welfare secretaries at three child welfare units in Skåne county, Sweden. Generated material was analyzed using Lipsky’s theory of street- level bureaucracy. Results showed that dental health is not routinely paid much attention to by professionals unless dental needs are serious or being brought up by a key actor. The complex role of the street-level bureaucrat, including the delicacy of intervening with families' lives as well as the need for professionals to prioritize other aspects of health needs due to high workload, was reported by secretaries as reasons to not routinely review the children’s dental health. Lack of protocol and knowledge among welfare secretaries concerning the dental health of the target group were found to reduce children's access to dental care. Finally, unclear arrangements between actors regarding the responsibility of the child’s dental health was found to add to the problem. The study emphasizes the need for further research on how professionals can identify and contribute to dental health needs of children subjected to the social services. (Less)
