Ansträngningsutlöst andningsfunktionsstörning drabbar främst barn och ungdomar, oftast i samband med idrottsutövning och är vanligare bland flickor än pojkar. Mekanismerna bakom besvären är komplexa och inte helt klarlagda. Enhetliga rutiner för omhändertagandet finns inte. Syftet med uppsatsen var att kartlägga och utvärdera omhändertagandet av personer med dessa besvär vid Röst- och Talvårdsavdelningen, Skånes Universitetssjukhus i Lund. I en retrospektiv studie analyserades 27 journaler från patienter som remitterats till avdelningen sedan 2004. 26 patienter erbjöds att delta i en uppföljande telefonintervju. 14 av dessa tackade ja till att medverka varav 3 vid intervjutillfället var besvärsfria och 8 hade hanterbara eller minskade... Det framgick av resultaten att rutiner för omhändertagandet har modifierats successivt under perioden 2004- 2011 och att det är önskvärt att ha mer enhetliga och fasta rutiner för utredning och behandling av denna patientgrupp. Vidare framkom behov av mer information som dessutom bör vara bättre anpassad till tonåringar och deras föräldrar. Då andelen patienter som gått i logopedisk terapi var låg kan inga slutsatser dras kring effektiviteten. Analysen visar dock att fler regelmässiga uppföljningsbesök än de som görs idag bör ingå i omhändertagandet. Det finns dessutom ett behov av vidare forskning om mekanismerna bakom ansträngningsutlöst andningsfunktionsstörning. Prospektiva studier är nödvändiga för att bedöma effekten av den logopediska behandlingen. (Less)

The mechanisms of the disorder are complex and not fully understood. Uniform clinical procedures for examination and treatment of these patients are not established. The purpose of this paper was to identify and evaluate the present care of this category of patients at the Department of Voice and Speech care in Skåne University Hospital in Lund, Sweden. Exercise-induced respiratory dysfunction affects mostly children and young people, usually during athletic performance, and is more common among girls than boys. The mechanisms of the disorder are complex and not fully understood. Uniform clinical procedures for examination and treatment of these patients are not established. The purpose of this paper was to identify and evaluate the present care of this category of patients at the Department of Voice and Speech care in Skåne University Hospital in Lund, Sweden. In the retrospective study 27 medical records of patients cared for at the department since 2004 were analyzed. 26 patients were then invited to participate in a follow-up telephone interview. 14 of these agreed to participate of whom 3 declared to be symptom-free and 8 had manageable or decreased symptoms, while 3 reported unchanged or increased breathing discomfort. The analysis of medical records showed that during the follow up period of 7 years (2004-2011) the procedures for exam and treatment were gradually modified and we can not draw any conclusions as to the possible effects of the speech pathology therapy. Furthermore, it appeared desirable to introduce uniform and fixed routines for evaluation and treatment of these patients. Need for more extensive and age suited information emerged. In addition, the analysis showed that an extended and more regular follow-up than that of today should be included in the care. There is a need for further research on the mechanisms of exercise- induced respiratory dysfunction. Prospective studies are required to assess the efficacy of the speech pathology treatment. (Less)
