The study was based on six semi-structured interviews with care professionals in the field of home care services. To analyse our empirical material, we have used Thomas Brante’s professional theory and Andreas Liljegren’s theory of boundary work which explains how the professions mark their boundaries. Our study has shown that users in the home care who suffer from a substance abuse problem prove to be a complex problem, partly because of the self-determination principle which is highly respected and partly that there are unclear responsibilities. The results revealed a paradoxical situation: Home care professionals’ considered treatment of substance abuse to be beyond their responsibility, but they also adjusted care services in order to make people with substance abuse reduce their alcohol consumption. Unclear responsibility may lead to suffering for older people with substance abuse not receiving the help they need, but also to ethical stress for home care professionals not knowing if they have done “the right thing” or if they should have done more.
