Bestående psykologiska konsekvenser av tidig omskärelse: en systematisk litteraturöversikt

Manlig omskärelse är det vanligast utförda kirurgiska ingreppet i världen och har en historia som sträcker sig tusentals år bak i tiden. Ingreppet kan genomföras under hela livet men sker i stor omfattning under tidig barndom. Syftet med detta arbete har varit att sammanställa en ämnesöversikt över kunskapsläget om möjliga psykologiska konsekvenser av ingreppet i tidig barndom. En systematisk litteraturöversikt genomfördes för att samla in och bedöma litteraturen. Av 890 unika studier för granskning bedömdes enbart fyra uppfylla studiens krav vilket var oväntat få. Ingreppet kan genomföras under hela livet men sker i stor omfattning under tidig barndom. Syftet med detta arbete har varit att sammanställa en ämnesöversikt över kunskapsläget om möjliga psykologiska konsekvenser av ingreppet i tidig barndom. En systematisk litteraturöversikt genomfördes för att samla in och bedöma litteraturen. Av 890 unika studier för granskning bedömdes enbart fyra uppfylla studiens krav vilket var oväntat få. En narrativ analys användes för att sammanställa och beskriva studieresultaten. Utifrån granskade studier finns det uppenbara kunskapsluckor. Studieresultat visar på samband till ett antal psykologiska fenomen men saknar metodologisk styrka för att kunna dra några säkrare slutsatser. Starkt vetenskapligt stöd finns för att tidiga smärtupplevelser kan resultera i bestående förändringar. Vilken grad av smärta som upplevs vid tidig omskärelse är dock inte tillräckligt studerat. Smärtlindrande åtgärder rekommenderas vid ingreppet, men flera studier har visat att ingreppet i viss omfattning genomförs helt utan smärtlindring. Utifrån litteratur som visar samband mellan tidiga smärtupplevelser och bestående psykologiska förändringar bör tidig omskärelse studeras utifrån premissen att det kan finnas signifikanta skillnader beroende om ingreppet genomförts med eller utan smärtlindring. Forskningsläget har kunskapsluckor som bör åtgärdas genom framtida studier med stark studiedesign som möjliggör tydliga slutsatser. (Less)

The procedure can be carried out throughout life but is to a large extent performed during infancy. Male circumcision is the most performed surgical procedure in the world and has a history that stretches back thousands of years. The procedure can be carried out throughout life but is to a large extent performed during infancy. The purpose of this study has been to compile a subject overview that clarifies the state of knowledge regarding possible psychological consequences of the procedure when performed in early childhood. A systematic literature review was conducted to collect and assess the literature, out of initial 890 unique studies gathered for further review only four were determined to meet the study's requirements, which was unexpectedly few. A narrative analysis was used to synthesize and describe the study results. Based on reviewed literature there appears to be obvious knowledge gaps, study results show correlation to several psychological phenomena but lack methodological strength to enable any strong conclusions. There is strong scientific support for early pain experiences causing lasting psychological changes, however, the degree of pain experienced during early circumcision has not been properly studied. Pain-relieving measures are recommended during the procedure, but studies have shown that it is to some degree performed without pain relief. Based on literature that shows a correlation between early pain experiences and permanent psychological changes, early circumcision should be studied with the premise that there could be significant differences depending on whether the surgery was performed with or without pain relief. The state of research has knowledge gaps that should be addressed through future studies using adequate study designs to enable strong conclusions. (Less)
