- Abstract
- The Swedish elderly care system has undergone a significant transformation towards marketization and New Public Management (NPM). This study aims to analyze how municipal structural factors, specifically economic resources, demographic volume, and organizational form, co-vary with user satisfaction in Home Care (hemtjänst) and Residential Care. The study employs a quantitative cross-sectional design, conducting a total population survey of Sweden’s municipalities (N = 286) using secondary data from 2024. The analysis combines Pearson's correlation coefficient with quartile comparisons to identify linear trends and threshold effects.

The study employs a quantitative cross-sectional design, conducting a total population survey of Sweden’s municipalities (N = 286) using secondary data from 2024. The analysis combines Pearson's correlation coefficient with quartile comparisons to identify linear trends and threshold effects.
The results indicate that economic resources have a negligible impact on user satisfaction. Instead organizational scale and operational form prove to be decisive factors. A clear dichotomy emerged between the two service forms: Home Care satisfaction is negatively correlated with municipality size and the degree of privatization. Conversely Residential Care shows a positive correlation with the degree of privatization. The strong negative correlation between privatization and satisfaction within Home Care challenges the efficacy of market mechanisms in services dependent on relational continuity. (Less)
