Psykos och mentalisering: Reflekterande funktion efter en första psykos

Mentalisering handlar i stora drag om hur vi förhåller oss till och förstår oss själva, andra och omvärlden. Psykos beskrivs påverka dessa områden, men sökningar efter studier som undersökt psykos och mentaliseringsförmåga gav få träffar. Syftet med studien var att undersöka mentaliseringsförmåga hos psykospatienter. För att undersöka förmågan utan påverkan av akuta symtom valdes patienter i remission efter en första psykos. Mentalisering handlar i stora drag om hur vi förhåller oss till och förstår oss själva, andra och omvärlden. Psykos beskrivs påverka dessa områden, men sökningar efter studier som undersökt psykos och mentaliseringsförmåga gav få träffar. Syftet med studien var att undersöka mentaliseringsförmåga hos psykospatienter. För att undersöka förmågan utan påverkan av akuta symtom valdes patienter i remission efter en första psykos. En intervju, Adult Attachment Interview (AAI), utfördes med 5 patienter för att bedöma mentaliseringsförmåga operationaliserad som reflekterande funktion (RF). Ett skattningsformulär för behandlare konstruerades för att undersöka i vilken utsträckning patientgruppen använder sig av reflekterande funktion i den behandlande kontakten. Resultatet av RF-skattningen ur AAI visade på en spännvidd mellan låg och ordinär reflekterande funktion, med ett medianvärde motsvarande låg funktion. Resultatet av de 12 behandlarnas skattning visade att patienterna använde sig av reflekterande funktion ibland i den behandlande kontakten. Urvalsgrupperna i studien är för små för att ge generaliserbara resultat. Mentalisering är en förmåga som kan utvecklas, exempelvis genom ett mentaliserande förhållningssätt i behandling. (Less)

The results of the RF-rating on the AAI showed a range from low to ordinary reflective functioning, the median was low RF. The result from the 12 clinicians´ ratings showed that the patients used RF sometimes in the clinical contact. Both samples are too small for generalization of the results. Mentalization is an ability that can be improved and elaborated, for example within the context of a mentalizing stance in treatment. (Less)
