- Abstract
- Preservatives and non-ionic surfactants are frequently used in biological drugs for multiple dosage.
However, the most commonly used surfactants and preservatives are known to be incompatible with each other,
leading to the solution becoming turbid due to precipitation.
This study aims to investigate at which concentration regions precipitation
occurs between non-ionic surfactants (polysorbate 80 and Dodecyl Maltoside)
and the most common preservatives (phenol, benzyl alcohol and m-cresol) used
in biological drug formulation. In addition, proteins (Somatropin and bovine
serum albumin) were added to see if they changed the behaviour of the solution.

Results confirmed that the precipitation of the surfactant was sensitive to the
addition of preservative, but also that concentration regions where they are
compatible with one another exists. The study also showed that the presence of
bovine serum albumin had no significant effect on the incompatibility regions,
whilst Somatropin in the same buffer and in the presence of NaCl, became turbid
upon addition of even the smallest amount of either phenol or m-cresol.
Another part of the study was to investigate if the protein was affected by the solution becoming turbid.
This involved analyses of the protein distribution within the two phases in a cloudy region of a non-ionic surfactant mixed with preservative.
It also included an investigation of potential changes of the secondary structure of the proteins as the solution becomes turbid.
The results from these studies were that bovine serum albumin had a slightly higher concen-
tration within the non-turbid region than the precipitate and on the contrary,
a significantly higher fraction of Somatropin was found in the precipitate.
These observations suggests that BSA had no interaction or preference towards the
other components, whilst Somatropin formed some sort of complex with the

Samtidigt behöver proteinet, som är den aktiva ingrediensen i ett biologiskt läkemedel, stabiliseras med olika typer av tillsatsämnen. Ett exempel är surfaktanter som används för att motverka att proteinet klumpar ihop sig eller fastnar på ytor. Dessvärre har man upptäckt att dessa två tillsatsämnen har låg kompabilitet med varandra och kan ge upphov till en fasseparation, som uttrycks i att lösningen blir grumlig. Konsekvenserna kan vara förödande, då det bland annat kan resulterar i en ojämn fördelning av proteinet i lösningen, som i sin tur kan leda till att en felaktig dos av läkemedlet administreras till patienten. En viktig sak att notera är att upphov till grumlighet är beroende av tillsatsämnenas koncentrationer. För att förhindra risken att patienter kommer till skada, så krävs en kartläggning kring vid vilka koncentrationer som dessa ämnen går att blanda utan att fasseparation uppstår, vilket var huvuduppgiften i denna studie.
Studien innefattade de vanligast använda surfaktanter och konserveringsmedel, och baserades på de mängder och under de förhållanden som är vanligast förekommande i biologiska läkemedel. Resultatet visade att det finns ett kompabilitetsområde vid en blandning av dessa tillsatsämnen inom de önskade koncentrationsintervallen. Vidare studier gjordes för att se om salt, som även det är en vanlig komponent i ett läkemedel, och om proteinet påverkar. Saltet visade sig försvåra blandning av läkemedel innehållande konserveringsmedel och surfaktanter, genom att minska koncentrationsområdena då dessa är kompatibla med varandra. Något överraskande resultat var att även att vissa protein bidrar till fasseparation om salt finns närvarande, vilket alltså minskar möjligheten för att skapa en jämnt fördelad lösning. Vidare, kunde man även se att i en grumlig lösning så har proteinet en ojämn fördelning vilket bekräftar relevansen av denna studie då detta kan leda till bristfällig administrering och således vara skadligt för patienten.
