Prevention of beta-cell dysfunction via targeting novel GPCRs in pancreatic islets

- The primary aim of this thesis was to investigate the role of G protein coupled receptors (GPCRs) in insulin secretion and beta-cell survival. The second aim was to determine which pathway is involved in insulin release and beta-cell protection via GPCRs.
This has been investigated in three different studies, which concluded the
following:
Study 1) GPR30 is expressed in female mice pancreatic islets. G-1 (GPR30
agonist) and estrogen stimulate insulin secretion and protect cytokine induced apoptosis of beta-cells even in the presence of nuclear receptor (ERalpha and ERbeta ) antagonists.

Study 1) GPR30 is expressed in female mice pancreatic islets. G-1 (GPR30
agonist) and estrogen stimulate insulin secretion and protect cytokine induced apoptosis of beta-cells even in the presence of nuclear receptor (ERalpha and ERbeta ) antagonists.
Study 2) The activation of GPR30 mediates insulinotropic effects mainly via
cAMP/PKA pathway, anti-apoptotic effects via phosphorylation and activation of CREB, AKT and ERK pathways on female human pancreatic islets. In addition, we have shown here that estrogen and G-1 potentiate the effects of glibenclamide (sulfonylurea) and abolish the inhibitory effects of clonidine on insulin secretion from female human pancreatic islets.
Study 3) Protease-activated receptor 2 (PAR-2) belongs to a subfamily of G
protein-coupled receptors and is expressed in pancreatic islets. Several studies have suggested that the PAR-2 is an important mediator of inflammation. PAR-2 is highly expressed in diabetic vs normal subjects and induction of PAR-2 expression via PAR-2 agonists (SLIKGV or tryptase) mediates apoptosis and reduces the rate of cell proliferation.
In this study we have shown that treatment with alpha-1 antitrypsin protects
against PAR-2 specific peptide induced apoptosis in human pancreatic islets via a MAP kinase pathway. A1AT displayed PAR-2 antagonistic activities and
suppressed the PAR-2 pro-inflammatory effects. In addition we have shown that the alpha-1 antitrypsin stimulates insulin secretion in a dose-dependent manner. In view of these novel findings we suggest that drugs that can selectively inhibit the activity of PAR-2 and activate GPR30 receptor will be of great benefit in the treatment of diabetes mellitus. Högt blodsocker gör att kroppens celler arbetar konstant, och om blodsockret är för högt under en längre tid förstörs kroppens celler och andra följdsjukdomar kan uppstå, som hjärt/kärl-, ögon-, njur- och nervsjukdomar.
De celler som producerar hormonet insulin finns i bukspottskörtel och kallas
för beta-celler. Insulin produceras efter en måltid och signalerar till kroppens muskel-, fett- och leverceller att ta upp och använda sockret i blodet.
I min forskning har jag kunnat påvisa att östrogen har en skyddande förmåga
för dessa celler samt att det får insulinproduktionen att öka. Östrogen är ett i huvudsak kvinnligt hormon men finns hos både kvinnor och män. Östrogen
fungerar som ett signalhormon i kroppen och aktiverar sina receptorer, s.k.
signalmottagare. I kroppens celler finns det två typer av östrogenreceptorer, yttre och kärnreceptorer. Den yttre receptorn jag studerat i både möss och människor kallas för GPER och skiljer sig från kärnreceptorerna. Fäster sig för mycket östrogen på kärnreceptorerna har östrogen negativa egenskaper, såsom att bröstcancerceller kan uppstå. När endast GPER signalerar har dock östrogenet positiva egenskaper. Detta har jag kunnat studera med hjälp av ett läkemedel kallat G-1, som gör att endast GPER är aktiverad.
G-1:s effekt på bukspottskörtelns celler från möss och människor är att
insulinproduktionen ökar samt att glukagonproduktionen minskar (glukagon
motverkar insulinets effekt). Dessutom överlevde tre gånger fler celler som
behandlats med G-1 när cellerna utsattes för dödliga faktorer.
Utöver dessa studier har jag påvisat alpha-1 antitrypsins, AAT:s, goda effekter på beta-celler. AAT är ett protein som förhindrar att signalen når den inflammatoriska receptorn PAR-2. Denna blir då inaktiverad, vilket leder till att fler celler överlever vid inflammation. Högt blodsocker under lång tid bidrar till ökad inflammation som förstör beta-cellerna och vid minskad inflammation kan beta-cellerna alltså överleva längre.
Resultatet från min forskning kan leda till att det kan tillverkas bättre
läkemedel mot diabetes framförallt för kvinnor men även för män. Under en
kvinnas liv förändras östrogennivån och diabetes kan därmed uppstå – G-1 och
AAT-liknande läkemedel kan därför i framtiden användas tillsammans med
dagens befintliga läkemedel för att behandla typ 1 och 2-diabetes. (Less)
