Antidepressant drug effects in vivo: Focus on pharmacokinetic and pharmacodynamic responses in different experimental paradigms

Abstract
Pharmacokinetic and pharmacodynamic activities of the antidepressants venlafaxine (VEN) and citalopram (CIT) were investigated in the portacaval shunted (PCS) rat, a model of chronic hepatic encephalopathy (HE), and normal/control rats. The levels of VEN in serum and brain were higher in PCS rats than in controls after a single injection and chronic treatment with VEN (10 mg/kg). After reducing the dose by 50% to PCS rats (i.e. 5 mg/kg, single injection), the levels of VEN were still higher in experimental HE than in controls. Furthermore, since the T1/2 for VEN was prolonged by 80% in experimental HE, the results suggest that liver-insufficient patients may have to be treated with both reduced doses and with longer dosing intervals than... The S/R ratio of the enantiomers of VEN differed between serum and brain in both experimental HE and controls, but the ratio was not altered in experimental HE versus controls. This could be reassuring regarding drug safety for VEN in patients suffering from HE. There were no major differences in S/R ratio of CIT between serum and brain of normal rats. The levels of 5-HT in brain dialysis samples were higher in the PCS rats compared to controls following administration of 10 mg/kg to both groups. However, the 5-HT levels were not higher in experimental HE compared to normal rats when the dose of VEN was reduced in PCS rats. In conclusion, both pharmacokinetic and pharmacodynamic alterations were observed in experimental HE after administration of the antidepressant drugs VEN and CIT. (Less)

Leverencefalopati (LE) är ett neuropsykiatriskt sjukdomstillstånd som kan ses i samband med en försämrad leverfunktion. Den bakomliggande orsaken till LE är ofta levercirrhos (skrumplever) p.g.a. alkoholmissbruk, men andra orsaker såsom kronisk hepatit, kirurgi, och olika gifter kan också ligga bakom detta. Vid levercirrhos försämras leverns blodflöde p.g.a. en ökad kärlresistens. Detta medför att kollateraler bildas vid sidan om blodkärlen och blodet shuntas runt/förbi levern. Perifera organ (t.ex. hjärnan) som är beroende av att levern metaboliserar främmande ämnen, får då en förändrad tillförsel av substanser, vilket kan leda till det tillstånd som kallas LE. Patienter med LE uppvisar liknande symptom som de som kan ses hos patienter med olika affektiva sjukdomar. Detta innebär i sin tur att patienter med LE mycket väl kan komma att behandlas med, t.ex. antidepressiva läkemedel som venlafaxin (VEN) och citalopram (CIT). Eftersom nedbrytningen av dessa läkemedel primärt sker i levern (vars funktion är försämrad vid LE) kommer troligen deras effekt på LE-patienter skilja sig från den effekt som fås i patienter med normal leverfunktion. De patogenetiska mekanismerna vid LE är ej utredda men ett antal teorier finns. En av dessa teorier kan vara en störning av omsättningen av monoaminer (främst signalsubstansen serotonin) i hjärnan. Vidare, eftersom antidepressiv behandling i sin tur påverkar monoaminomsättningen är det svårt att förutsäga hur antidepressiv behandling skulle påverka LE-patienter. När PCS råttor och kontroller fick samma dos VEN sågs en klart högre koncentration av VEN i serum, hjärnparenkym och hjärndialysat i PCS råtta. För VEN finns en rekommendation att till patienter med försämrad leverfunktion sänka dosen med 50%. Trots detta förfarande så var fortfarande VEN koncentrationen högre i serum och hjärna hos PCS råtta. Vidare sågs också en längre halveringstid för VEN hos PCS råtta vilket talar för att dosen inte bara skall halveras utan även ges med längre tidsintervall vid försämrad leverfunktion.När råttor behandlades en längre tid (kroniskt) med VEN var koncentrationen av VEN fortfarande högre i PCS. Båda enantiomererna av VEN fanns också i högre koncentration i PCS än i kontroller. Kvoten mellan enantiomererna för VEN (den s.k. S/R-kvoten) skilde sig åt mellan serum och hjärna, dock var det ingen skillnad i kvot mellan PCS och kontroller. Det var ingen större skillnad på S/R-kvoten för CIT i serum och i hjärna. Förhöjda nivåer av serotonin, sågs hos PCS råtta efter akut behandling med samma VEN dos som för kontroller. Vid halvering av VEN-dosen till PCS råtta kunde inte längre denna skillnad i serotoninnivå observeras, vilket talar för en halvering av dosen VEN vid nedsatt leverfunktion. Inga tydliga effekter sågs på råttornas beteende efter kronisk behandling med VEN och CIT. En liten aktivitetsökning kunde bara observeras under första nattens beteendetest efter VEN-behandling. Inga samband mellan koncentrationen av VEN och effekten av VEN i kroppen (koncentrations-effekt samband) kunde påvisas hos råtta efter kronisk behandling med VEN. (Less)
