- This dissertation examines the interaction between municipal home-helpers and care re-cipients from a social psychological perspective. One of the starting points is a critique of some classical works in the caring science inspired by women’s studies for not taking into consideration that care-giving work constitutes social interaction. The aim is to analyse how elderly people and the home-helpers perceive and interpret each other, what they expect of their mutual relationship, how they influence each other’s actions in their emotional interaction, and how the old person’s family and the home-helper’s work team influence their relationship. The empirical investigations, analysed with qualitative methodology, comprise thematically structured in-depth interviews with 48 home-helpers in six work groups, and 40 case studies of assistance cases, based on interviews with the old person (25 cases) and participant observation of the parties (15 cases). The home-helpers and the old people perceive their relationship as instrumental, friendly, emotional, insecure (recipients), laborious or charged with conflict (home-helpers). It is shown that what the old person and the home-helper expect of their emotional relationship is linked to social needs such as respect, acknowledgement, and appreciation from the other. These aims influence the experiences and actions of the two parties and, thereby, the care-giving work. Commonly, the home-helper subordinates herself to the old person or adapt to the recipient, e.g. by listening and “adjusting” to the old person. Psychological constructions play an important role in the home-helpers’ perceptions of the care situation in emotional conflicts. In escalated conflicts the home-helpers may act more impersonally and showing less care in the work. The old people may express confirmatory feelings for the home-helpers, deepen their dependence, and towards the home-helper they represent in various ways who they are and what they stand for. The old persons’ conflict actions can be understood as attempts to value themselves positively. (Less)

Den omsorg hemtjänsten står för spelar en viktig roll för de äldre när det gäller att kunna leva ett gott liv i hemmet om de drabbas av sjukdom eller skröplighet. Hemtjänsten har en stor betydelse för äldres och deras anhörigas hälsa och livskvalitet.
I avhandlingen beskrivs hur den äldre och vårdbiträdet upplever och tolkar varandra, hur de uppfattar sin inbördes relation och hur de påverkar varandras tankar och känslor. Det visas på vilken dynamiken är i deras känslomässiga förhållande och hur denna inverkar på deras handlingar.
Vårdbiträdet kan anpassa sig till vårdtagaren bl a genom att lyssna in och ”ställa om sig” till den äldre och spela olika roller. Anpassningen och underordningen kan förstås som led i byggandet av relationen till den äldre. Vårdbiträdet skapar psykologiska konstruktioner av vårdtagaren och omsorgsarbetet bl a som ett led i hanteringen av konflikter i det känslomässiga förhållandet till vårdtagaren. Konstruktionerna ger vårdbiträdets handlande ett berättigande genom den mening som tillskrivs den äldres handlingar. När konflikter med den äldre trappas upp, kan vårdbiträden uttrycka en negativ värdering av den äldre genom att uppträda mer opersonligt i sitt handlande och vara mindre omsorgsfull i sitt arbete.
Ett sätt att förhålla sig till det beroendet till vårdbiträdena är att vara följsam i beroendet. De äldre uttrycker bekräftande känslor för vårdbiträdena, fördjupar sitt beroende och gestaltar på olika sätt vem man är och vad man står för inför vårdbiträdena. De äldre kan sträva efter att vara något i relationen till vårdbiträdet. Förväntningar hos den äldres på att bli respekterad och värderad positivt uppfylls. Ett annat förhållningssätt till vårdbiträdena är att sträva efter frigörelse från beroendet. Hon kan försöka klara sig utan hjälp, utöva inflytande och kontroll eller hävda sin integritet gentemot vårdbiträdena.
De äldres och vårdbiträdenas förväntningar på deras känslomässiga förhållande tycks sällan leda till öppna konflikter till följd av att dessa förväntningar inte tillfredsställs. Detta tyder på att de äldre i allmänhet känner sig respekterade av vårdbiträdet och vårdbiträdena upplever uppskattning och erkännande i omsorgsarbetet. En process i det tysta äger rum i samspelet mellan vårdtagaren och vårdbiträdet, där man förhandlar med varandra och anpassar sig till varandra. Samspelet kan bli konfliktladdat när den äldre och vårdbiträdet inte lyckas anpassa sig till de inbördes förväntningarna på mötet. När samspelet är konstruktivt upplever den äldre och vårdbiträdet karaktären i sitt förhållande som vänskapligt.
Den äldres och vårdbiträdets samspel kan relateras till generella aspekter av mellanmänskligt samspel. En central dynamik i vårdtagarens och vårdbiträdets känslomässiga samspel kretsar kring behov av att få respekt av andra och att ens individualitet blir känd och uppskattad. Den äldres och vårdbiträdets förväntningar på deras känslomässiga förhållande är kopplade till dessa sociala behov. De förväntar sig respekt, erkännande och uppskattning av den andra. Dessa mål påverkar de två parternas upplevelser och handlingar. Strävanden i och kring omsorgsarbetet och strävanden kring det egna självet är på detta sätt två aspekter av handlandet som är sammansmälta i den äldres och vårdbiträdets samspel. (Less)
