Abstract
Background: Endometriosis is one of the most common causes of infertility in women. Despite extensive research on the subject, early-stage diagnosis remains challenging. Histologically, normal endometriosis does not differ from the eutopic endometrium, shifting the focus of investigation to the molecular level.

Background: Endometriosis is one of the most common causes of infertility in women. Despite extensive research on the subject, early-stage diagnosis remains challenging. Objective: To assess the immunohistochemical markers ER, PGR, Ki-67, BCL-2, and MMP-9 in normal endometrial, eutopic, and ectopic endometrial tissue in women with endometriosis and infertility.
Methods: The study divided samples into three groups based on the expression of the investigated markers in glands (ER, PGR, Ki-67, BCL-2, MMP-9) and stroma (ER, PGR, Ki-67). Strict inclusion and exclusion criteria (clinical and/or morphological) were applied to organize these groups. Immunohistochemical staining was assessed in at least 10 fields of view per sample. Statistical analysis included nonparametric tests.
Results: Significant differences were observed between normal endometrium and both eutopic and ectopic endometrium for ER, PGR, and MMP-9 expression. Additionally, BCL-2 and Ki-67 showed significant differences between normal and eutopic endometrium (p < 0.05 for all markers). However, no statistically significant differences were found between eutopic and ectopic endometrium.
Conclusion: The combined use of ER, PGR, and MMP-9 immunohistochemical markers can aid in diagnosing endometriosis in women with infertility. The inclusion of BCL-2 and Ki-67 may further enhance diagnostic accuracy. (Less)
