Ghrelin and CART. Two new islet peptides. Expression, regulation and function.

- Until recently, the islets of Langerhans were thought to harbour four main cell types characterised with respect to principal hormone product. Here we show that the novel hormone ghrelin is expressed in a fifth islet cell type, the islet ghrelin cell, in humans and rodents. The cells are developmentally regulated in that they are numerous around birth and fewer in the adult. The islet ghrelin cells have unique ultrastructural features that distinguish them from the other islet cells. Further, we demonstrate that ghrelin inhibits GSIS from clonal beta cells. The islet ghrelin cells have unique ultrastructural features that distinguish them from the other islet cells. Further, we demonstrate that ghrelin inhibits GSIS from clonal beta cells. In addition, we show that the neuropeptide CART is expressed in several islet cell types during rat development, but only in delta cells and nerves in normal adult animals. Further, CART is robustly upregulated in the beta cells of several rodent models of type-2 diabetes. We also show that CART augments cAMP-mediated GSIS from clonal beta cells and from isolated rat islets via increased intracellular cAMP and the PKA-dependent pathway. On the other hand, in the absence of cAMP-elevating agents CART inhibits secretion of insulin, glucagon and somatostatin. Further emphasizing a role for CART in the islets, CART KO mice display impaired GSIS in vivo and in vitro together with impaired glucose elimination and reduced expression of beta cell function markers. Although CART KO mice have normal food intake they develop a late onset increased body weight. Importantly, islet dysfunction is apparent already in young, still lean animals. (Less)

Tills nyligen trodde man att öarna bestod av fyra celltyper; insulinproducerande beta celler, glukagonproducerande alfa celler, somatostatinproducerande delta celler och PP-producerande PP-celler.
Ghrelin:
Ghrelin är ett nyupptäckt hormon som påverkar frisättning av tillväxthormon, ätbeteende och ämnesomsättning. Ghrelin bildas främst i magsäcken, men vi har nu funnit att ghrelin även produceras i en ny celltyp, ghrelincellen, i öarna. Denna utgör en femte ö-celltyp hos människa, råtta och mus. Ghrelincellerna i pankreas är mycket talrika under foster- och neonatalutvecklingen, och utgör då hela tio procent av alla ö-celler hos människa. Ghrelincellerna finns även i öarna hos vuxna människor och utgör då ca 1% av alla ö-celler. Vi har även visat att ghrelin hämmar glukosstimulerad insulin sekretion från odlade insulin-producerande celler. Funktionen av pankreatiskt ghrelin kan således vara att skydda fostret från för lågt blodsocker genom att hämma insulinsekretion. Alternativt spelar det en roll i regleringen av tillväxthormonfrisättning under fosterutvecklingen, då magens ghrelinceller fortfarande är få.
CART:
CART är en peptid som finns i nerver i de centrala-, perifera- och enteriska nervsystemen. CART har även påvisats i endokrina celler i hypofysen, binjurenmärgen och i öarnas delta celler. Vi har funnit att hos råtta finns CART i alla ö-celltyper, förutom ghrelinceller, kring födelsen. Vi har även funnit att CART är en viktig regulatorisk peptid i öarna. CART ökar insulinsekretion stimulerad med tarmhormonet GLP-1. Detta har vi visat i cellinjer och i försök på isolerade råttöar. Vi har även funnit en del av mekanismen bakom denna effekt. CART ökar produktionen av cAMP (en viktig budbärarmolekyl) och ökar därmed aktiviteten av enzymet proteinkinas A. Dessutom visar vi att möss som saknar CART har gravt försämrad insulinsekretion tillsammans med glukosintolerans. Denna slutsats grundar sig på både försök i levande djur och försök på isolerade öar. Intressant nog uppvisar människor med en naturlig mutation i CART genen en ökad risk för typ-2 diabetes. I studier av flera olika råttmodeller för typ-2 diabetes har vi funnit att CART är kraftigt uppreglerad i beta cellerna hos dessa. Vår slutsats är att frånvaro av CART leder till utveckling av typ-2 diabetes och att CART uppregleras i beta cellerna när ökad insulinproduktion krävs. Dessa data tillsammans tyder på att CART har en viktig roll i reglering av ö-funktion, glukosreglering och därmed vid uppkomsten av typ-2 diabetes.
