- The study was conducted to investigate whether cervical human papillomavirus infections (CHPI) are associated with contraceptive use and reproductive history. The contraceptive and reproductive histories in 972 women seeking contraceptive advice were noted and screening conducted for human papillomavirus infection. The interview included number of pregnancies and childbirths, legal and spontaneous abortions, and menstrual pattern. Information about current use of contraceptive methods, about casual sex, and history of combined oral contraceptive pill (OC) use was obtained. Women with a history of spontaneous abortion showed a significant correlation with CHPI, as did women who used high-dose OCs when compared with the remaining study population (odds ratio 3.0). There was no association between use of low-dose OCs and CHPI. In multifactorial analyses with adjustment for age, number of lifetime sexual partners, number of partners during the preceding six months and age at first intercourse, the significant correlation between use of high-dose OCs and CHPI remained (adjusted odds ratio 2.8). The results indicate a relationship between female steroid hormones and the occurrence of CHPI. An association with high-dose OCs could not be excluded. (Less)

Cette étude a été réalisée pour déterminer si les papillomatoses virales du col sont associées à l'utilisation des contraceptifs et aux antécédents de reproduction. Après avoir enregistré les antécédents de contraception et de reproduction de 972 femmes ayant demandé des conseils en matière de contraception, des examens ont été conduits pour dépister toute infection de papillomatose virale humaine. Les questions posées concernaient le nombre de grossesses et d'accouchements, d'avortements légaux et spontanés, et le régime des menstruations. On a également obtenu des renseignements sur l'utilisation courante de méthodes contraceptives, les rapports sexuels fortuits et les contraceptifs oraux combinés précédemment utilisés. On a constaté une corrélation significative entre papillomatose virale et non seulement avortement spontané mais aussi l'emploi de contraceptifs oraux à forte dose, par comparaison avec le reste de la population étudiée (rapport=3,0). Aucune association de ce genre n'est apparue avec l'utilisation des contraceptifs oraux à faible dose. Les analyses multi-factorielles, ajustées sur l'âge, le nombre de partenaires sexuels pendant la vie, le nombre de partenaires sexuels au cours des six mois précédents et l'âge au moment des premiers rapports sexuels, ont montré qu'il subsistait une corrélation significative entre contraceptifs oraux à forte dose et infection de papillomatose virale (rapport ajusté=2,8). Les résultats indiquent une relation entre les hormones stéroïdes femelles et la manifestation de papillomatose virale. Une association avec les contraceptifs oraux à forte dose n'est pas à exclure.
Abstract in Spanish
Se realizó un estudio a fin de investigar si las infecciones cervicales por el virus papiloma humano (CHPI) están asociadas con el uso de anticonceptivos y con la historia clínica en materia de reproducción. Se registró la historia clínica en materia de reproducción. Se registró la historia clinica anticonceptiva y reproductiva de 972 mujeres que solicitaban asesoramiento anticonceptivo, a quienes se examinó a los efectos de determinar la presencia de infecciones por el virus papiloma humano. La entrevista incluyó el número de embarazos y nacimientos, abortos legales y espontáneos, y modalidad de la menstruación. Se obtuvo información acerca del uso actual de métodos anticonceptivos, también en actos sexuales fuera de una pareja estable, y la historia del uso de anticonceptivos orales (AO) combinados. Las mujeres con antecedentes de abortos espontáneos sencaronalaron una correlación significativa con el CHPI, al igual que las mujeres que tomaban AO de dosis alta en comparación con la restante población del estudio (relación de probabilidad 3,0). No hubo ninguna asociación entre la administración de AO de dosis baja y el CHPI. En análisis multifactoriales ajustados según edad, número de parejas sexuales durante toda la vida, número de parejas en los seis meses anteriores y edad al primer acto sexual, se mantuvo la correlación significativa entre la administración de AO de dosis alta y el CHPI (relación ajustada de probabilidad 2,8). Los resultados indican una relación entre las hormonas esteroides femeninas y la presencia del CHPI. No pudo excluirse una asociación con los AO de dosis alta. (Less)
