A displaced femoral neck fracture in a person around retirement age, with some comorbidities, poses challenges in determining the optimal surgical treatment. The long-term result will be affected by the patient's overall health, their life style factors, and the patient's compliance with restrictions. Ideally, the surgeon has to foresee the vitality of the patient during his/her remaining life span. If the patient will survive several decades with high functional demands, an internal fixation may be the best choice. A slightly higher biological age, but still an active lifestyle will speak in favor of total hip arthroplasty.
